Enclosed is the ASME Section XI Code Case N-532-4 OAR-1 Owner's Activity Report for the 1 R25 Refueling Outage. There are no items to report for Table 1, "Items with Flaws or Relevant Conditions that Required Evaluation for Continued Service." Repair/Replacement activities did occur during Cycle 25 and are addressed in Table 2, "Abstract of Repairs, Replacement, or Corrective Measures Required for Continued Service." This report is for the second period of the 4th Interval lSI activities (Interval 4, Period 2, Outage 2). This letter contains no NRC commitments.

II PSW piping found to be leaking during G-Scan activities.

Replaced Section of Unit 1 Div. II PSW piping located in the Valve Box, found to be leaking during tape coat removal of underground piping. Due to the inability to isolate thru-wailleakage in a 30" PSW piping elbow, a temporary non-Code replacement was implemented per Temp Mod 1-10-023 on July 30,2010 (WO 1101202105).

The non-Code replacement consisted of leaving the defect (thru wall leakage) in the elbow and welding an assembly of a small section of 6" piping with a flanged connection and drain valve with a threaded pipe cap over the leak location.

Due to this being a non-Code replacement, HNP-ISI-Al 10 was submitted to the NRC for approval of the proposed replacement which had been prepared in accordance with NRC Gl90-05. Proposed non-Code replacement performed in accordance with B31.1, 2007 Edition. Acceptable per IWA-4224.1(a).

A Code Repair was performed during the lR25 outage.
