SUMMARY

MAY 1980 The plant was shutdown for the entire reporting period for an extended refueling outage. Major activities during the month included installation of the core spray sparger clamps, Installation of the reactor Internals and reactor head, '

and performance of the 10 year code hydrostatic test on the reactor vessel and coolant piping.

Four Reportable Occurrences were identified during the reporting period:

R.O. #80-17 was identified on May 16, 1980 when the one rod Free rod withdrawal block was found to be defeated for three control rod drives by two jumpers and a defective position Indicating probe.

R.O. #80-18 was identified on May 7,1980 when the "as found" setpoint for Reactor High Pressure Scram sensor RE030 was found to be greater than the allowable limit driving performance of routine surveillance test.

R.O. #80-19 was identified on May 17, 1980 when core spray relief ,

valves were discovered to be leaking through during testing as part of 1 j the leak reduction program.

I R.0. #80-21 was identified on May 20, 1980 when it was determined the surveillance required by Technical Specification 4.9.D was not being perfo rmed.
