Enclosed you will find two (2) Condition Report (CR) listings related to your request for previous Davis-Besse drill, exercise, and other "training opportunities" related to Davis-Besse's emergency response functions since the May 17,2005 Evaluated Exercise (Inspection Report 2005-011). The package contains a hard copy of the CR listings and also an "electronic copy" provided on a Compact Disc (CD) readable with Adobe Acrobat.
One (1) CR listing contains CRs categorized as Significant Conditions Adverse to Quality (SCAQ) and Conditions Adverse to Quality (CAQ). Davis-Bases' Corrective Action Program procedure defines SCAQ/CAQ CRs as Adverse Conditions that are: Failures, malhctions, deficiencies, deviations, defective hardware and non-conformances, or human performance, programmatic, organizational, or management weaknesses that adversely affect Quality(Q), Augmented Quality(AQ), or nuclear safety related equipment, programs, or processes.
The other CR listing contains CRs categorized as Conditions Not Adverse to Quality (NCAQ),
whch don't rise to the level of significance of a CAQ. This NCAQ listing contains items such as "procedure enhancements" and EOF/TSC equipment problems (ex., fax machine not working). We wanted to give you the opportunity to review thls NCAQ CR listing to decide if you wanted to review any of the NCAQ CRs in detail.
