[gj Plant: D I will explain the initial conditions, which steps to simulate or discuss, and provide initiating cues. When you complete the task successfully, the objective for this job performance measure will be satisfied.
Initial Conditions:
- 1. A reactor startup is in progress on Unit-1, OP-2 at step 6.6.G. 2. Shutdown CEAs are fully withdrawn
- 3. Regulating group CEAs are being withdrawn per OP-2 (There has been no rod motion for several minutes) 4. RCS Boron Concentration after completion last CEA withdrawal was 1690 PPM 5. Neutron counts unexpectedly begin to rise (30 CPS over the last five minutes) 6. The "S/D MON HI" annunciator (1C05, window D-33) alarms 7. 11 Purification Ion Exchanger has been in service for three weeks 8. You are performing the duties of the Unit-1 CRO Initiating Cue: The CRS directs you to respond to the reactivity excursion per the appropriate procedure.

OP-2, Plant Startup From Hot Standby To Minimum Load AOP-lA, Inadvertent Boron Dilution Time critical task: No Validation Time: 10 minutes Simulator Setup: 1. IC-8, with reactor subcritical, a startup in progress with only the shutdown CEAs withdrawn, and power in the CPS range. 2. Connect SDM and reset alarms. 3. 4. Set up the 1/M trending plot, on the Plant Computer, using OI-50A. Use System Lineup to change VCT concentration to 0. Modify the file to change the VCT concentration back to 897, but don't execute the file. It is not needed during the AOP to stop the dilution since the operator will fast borate. But changing the file back to 897 restores the simulator to its normal setup. 5. Run simulator until count rate begins to increase and a SDM alarm comes in, then freeze.
Appendix C Job Performance Measure Worksheet Form ES-C-1 ELEMENT (shaded+ * =CRITICAL STEP) CUE: Have simulator driver place Simulator in RUN TIME START: ---D Identify and locate AOP-IA,Section VI D VI.A. I. -Stop any outward CEA motion. D VI.B.1. -Stop any makeup to the RCS. D VI.C.1. -IF the rate of power rise can NOT be stopped, THEN perform the following actions: D VI.C.1.a.
-Trip the Reactor. D VI.C.1.b.
-IMPLEMENT EOP-0, POST TRIP IMMEDIATE ACTIONS. D VI.C.2. -IF, due to dilution, the Reactor approaches criticality below Zero Power Transient Insertion Limit, 54.4 inches on Group 3, THEN perform the following actions ... D VI.C.3. -IF, due to dilution, the Reactor approaches criticality above the Zero Power Transient Insertion Limit, 54.4 inches on Group 3, THEN perform the following
... D VI.C.4. -IF count rate is slowly increasing, due to the dilution, THEN perform the following actions to control the count rate: CUE: CRS directs that CEAs not be inserted, at this time. D VI.C.4.a.
-Insert CEAs as necessary.
STANDARD Same as element Directs RO to stop any CEA withdrawal.
Verifies CVC-512 is shut and RC MIU Pps are stopped. Places HS-2520 in BYPASS and checks position indication for CVC-520. Checks WRNI indications to determine rate of power rise, at 1 COS. Determines Reactor trip not required.
Monitors WRNI power, SUR, SDM, 1/M to determine status of criticality and determines step is NIA. Monitors WRNI power, SUR, SDM, 1/M to determine status of criticality and determines step is N/ A. Determines step is applicable.
No action required.
CUE: After BA Pumps run 1 minute, CPS start to decrease.
D VI.C.4.b.
-Borate the RCS as necessary.
Determines step is applicable.
