Valid Test Failures Division 1 & 2 Diesel Generators

Attached is a Special Report concerning Perry Unit I diesel generators. Two valid tests and failures are identified on each of the Division 1 and Division 2 diesel generators.

SPECIAL REPORT Division 1 and 2 Diesel Generator Valid Failures On February 27, 1987, a failure in the control air systm resulted in valid tests and failures of the Perry Plant Unit 1 Division 1 and 2 Diesel Generators.

On March 11, 1987, the Unit 1 Division 1 Diesel Generator experienced a valid test and failure as a result of a faulty control air mgulator. Additionally, on March 17, 1987, the Unit 1 Division 2 Diesel Generator experienced a valid test and failure as a result of a faulty control air regulator. The following discussion relates to these referenced valid test failures.

Division 1 Diesel Generator Valid Test Failure on February 27, 1987 On February 27, 1987, a valid failure occurred on the Division 1 Diesel Gererator while verifying operability per SOI-R43 " Division 1 and 2 Diesel Generator System Operating Instructions". This is the first valid failure to occur on Perry Unit 1, Division 1 Diesel Generator. As of February 27, 1987, the Division 1 Diesel Generator had undergone thirteen (13) valid tests, including this first valid failure.

The failure occurred while starting the Division 1 Diesel Generator per SOI-R43 to verify operability. During the start sequence, a defective control air solenoid valve created an undue dmand of the control air system which resulted in insufficient control air pressure to allow for the generator field flashing and autmatic start of the rocm ventilation and the emergency service water systems. The support systems were manually started and the Division 1 Diesel Generator was manually shutdown. To correct this malfunction, the faulty solenoid valve (Delaval Part F-586-061: liumphrey Products Model

'IOG2E 1-3-10-35) was replaced. The Division 1 Diesel Generator was restarted and verified operable by performing the Division 1 Diesel Generator Start and Load Surveillance Test. The Division 1 Diesel Generator was declared inoperable on February 27, 1987 at 1708 and was subsequently declared operable on February 28,1987, at 0135, for a total out-of-service time of 8 hours9.259259e-5 days <br />0.00222 hours <br />1.322751e-5 weeks <br />3.044e-6 months <br /> and 27 minutes.

Division 2 Diesel Generator Valid Test Failure on February 27, 1987 On February 27, 1987, a valid failure occurred on the Division 2 Diesel Generator while performing the Division 2 Diesci Generator Start and Load Surveillance Test. This is the fourth valid failure to occur on Perry Unit 1, Division 2 Diesel Generator. As of February 27, 1987, the Division 2 Diesel Generator had undergone twenty-eight (28) valid tests, including this fourth valid failure.

The failure occurred while starting the Division 2 Diesel Generator per SVI-R43-T1318 in response to Technical Specification 3.8.1.1 Action Statment B.

During the start sequence, a defective control air solenoid valve created an undue demand on the control air system which resulted in insufficient control air pressure to allow for the generator field flashing and autmatic start of the rom ventilation and the emergency service water systems.

The Division 2 Diesel Generator tripped on overspeed because the mechanical i governor was improperly adjusted to maintain speed at 470 rpn. To correct  !

this malfunction the faulty solenoid valve (Delaval Part F-586-061: Ilumphrey Products Model 'IOG2El-3-10-35) was replaced. The Division 2 Diesel Generator

m Page 2 was restarted and verified operable by performing the Division 2 Diesel Generator Start and Load Surveillance Test. The Division 2 Diesel Generator was declared inoperable on February 27, 1987 at 1905 and was subsequently declared operable on February 28,1987 at 0345, for a total out-of-service time of 8 hours9.259259e-5 days <br />0.00222 hours <br />1.322751e-5 weeks <br />3.044e-6 months <br /> and 40 minutes.

M7FE: Subsequent to this event the mechanical governor was properly re-adjusted. The previous adjustment was not considered as a factor in operability.

Division 1 Diesel Generator Valid Test Failure on March 11, 1987 On March 11, 1987, a valid failure occurred on the Division 1 Diesel Generator while performing a pre-start inspection air rol? per SOI-R43 " Division 1 and 2 Diesel Generator Systern Operating Instructions". This is the second valid failure to occur on Perry Unit 1, Division 1 Diesel Generator. As of March 11, 1987, the Division 1 Diesel Generator had undergone fifteen (15) valid tests, including this second valid failure.

The failure occurred while exercising the Overspeed Trip Lever on the Division 1 Diesel Generator per SOI-R43. Upon resetting the Overspeed Trip Lever the control air regulator failed to produce the required output of 60 psig.

The regulator could only produce an output of 10 psig. This resulted in a control air system loss of function and therefore a valid failure. To correct this malfunction the faulty air regulator (Delaval Part F-579-061:

Bellofram Corporation 241-960-069, Type 50) was replaced. The Division 1 Diesel Generator was restarted and verified operabic by performing the Division 1 Diesel Generator Start and Load Surveillance Test. The Division 1 Diesel Generator was declared inoperable on March 11, 1987 at 1015 and was subsequently declared operable on March 12,1987 at 0607 for a total out-of-service tune of 19 hours2.199074e-4 days <br />0.00528 hours <br />3.141534e-5 weeks <br />7.2295e-6 months <br /> and 52 minutes.

Division 2 Diesel Generator Valid Test Failure on March 17, 1987 On March 17, 1987 c valid failure occurred on the Division 2 Diesel Generator while in Standby Readiness. This is the fifth valid failure to occur on perry Unit 1, Division 2 Diesel Generator. As of March 17, 1987, the Division 2 Diesel Generator had undergone thirty-two (32) valid tests, including this fifth valid failure.

The failure occurred while the Division 2 Diccol Generator was in Standby Readiness. The control air regulator failed at the gasketed connection i between the regulator body and the dripwell. The gasket blew out causing )

a ecnplete loss of control air systm functionability. The problem appeared 1 to be related to the dripwell dimensions with respect to the regulator body I which resulted in insufficient clamping force on the gasket. To correct l this nalfunction the faulty dripwell (Delaval part F-579-061: Bellofram )

Corporation 241-960-069, Type 50) was replaced. The Division 2 Diesel l Generator was restarted and verified operabic by performing the Division l 2 Diccol Generator Start and Imd Surveillance Test. The Division 2 Diesel l Generator was declared inoperable on March 17, 1987 at 2253, and was subsequently l declared operabic on March 18,1987 at 0710 for a total out-of-service time of 8 hours9.259259e-5 days <br />0.00222 hours <br />1.322751e-5 weeks <br />3.044e-6 months <br /> and 17 minutes.

Page 3 Overall Corrective Actions A Licensee Event Report (LER) will be submitted by March 27, 1987, to address the failure of the Division 1 and 2 Diesel Generators. The LER will also discuss any needed program improvements involving the ilumphrey Products solenoid valves.

The surveillance testing interval will be decreased to once every 7 days, which is accordance with Technical Specification Table 4.8.1.1.2-1, for both the Division 1 and 2 Diesel Generators.
