,e e-o Attschment PY-CEI/01E-0257 L SPECIAL REPORT Division 3 Diesel Generator Valid Failure On November 12, 1986, a valid failure of the Division 3 Diesel Generator occurred while performing Surveillance Technical Specification 4.8.1.1.2.
This was the second valid failure to occur on the Unit 1 Division 3 Diesel Generator. As of November 12, 1986, the Division 3 Diesel Generator had undergone eight valid tests, including this valid failure.
The failure occurred while the Division 3 Diesel Generator was being loaded.
During the test, the governor speed setting motor (Manufacturer-Bodine; Woodward Governor Part Number 03040-200) failed and resulted in the inability to control load. The speed setting motor could not drive the governor to any
+ tting other than 200 kw.
Technical Specifications require the Division 3 sel Generator to be loaded at 2600 kw for greater than one hour. To correct malfunction, the speed setting motor was replaced and the circuit related to the operation of the speed setting motor was troubleshot. The failure was identified as an indicated open circuit in the speed setting motor windings.
No other problems were identified. Both the generator set supplier and the governor manuf acturer were contacted to determine if problems with the speed setting motor have been previously identified. Cause of the failure is believed to be an isolated case. Following motor replacement, the Division 3 Diesel Generator was restarted and verified operable by performing the Division 3 Diesel Generator Start and Load Surveillance Test.
The Division 3 Diesel Generator had been previously removed fron service and declared inoperable on November 2,1986 at 0451 for maintenance and was subsequently declared operable on November 17, 1986 at 0500. The total out-of-service time for the Division 3 Diesel Generator was 15 days and 9 minutes. Since the Diesel Generator was not in service at the time of failure, the total out-of-service time attributed to the failure was 5 days, 3 hours3.472222e-5 days <br />8.333333e-4 hours <br />4.960317e-6 weeks <br />1.1415e-6 months <br />, and 44 minutes.
The Division 3 Diesel Generator Surveillance Test interval has been changed from once every thirty-one days to once every seven days in accordance with Technical Specification Table 4.8.1.1.2-1.
