f<3 SPECIAL REPORT

  • Division 3 Diesel Generator Valid Failure On September 11, 1986, a valid failure of the Division 3 Diesel Generator-occurred while performing Technical Specification Surveillance 4.8.1.1.2. This was the first valid failure to occur on the Perry Unit 1, Division 3 Diesel Generator. As of September 21, 1986, the Division 3 Diesel Generator had undergone five valid tests, including this one valid failure.

The failure occurred while the Division 3 Diesel Generator was accelerating to rated speed and voltage. During the test, the governor pre position limit switch assembly (Micro Switch Part No. 11SM3-T) failed to properly reset the governor speed setting motor. Fourteen seconds into the test, the generator frequency was 61.3 Hz. Technical Specifications require that frequency be stablilized and maintained between 58.8 - 61.2 Hz at 13 seconds. The Division 3 Diesel Generator returned to a stable 61.04 Hz and was successfully loaded to 100% load for greater than I hour. To correct the malfunction the pre position limit switch assembly was replaced and its associated cams re-adjusted. The Division 3 Diesel Generator was restarted _and verified operable by performing the Division 3 Diesel Generator Start and Load Surveillance Test.

The Division 3 Diesel Generator was declared inoperable on September 21, 1986 at 2015, and subsequently declared operable on September 23, 1986 at 1330 for a total out-of-service time of I day, 17 hours1.967593e-4 days <br />0.00472 hours <br />2.810847e-5 weeks <br />6.4685e-6 months <br />, and 15 minutes.

The surveillance testing interval will remain at once every 31 days in accordance with Technical Specification Table 4.8.1.1.2-1.
