  • Refusal to Test - Licensee Employees An employee's urine specimen was noted to have been dilute at the time of a random test collection. The specimen was then sent to an approved confirmatory laboratory for testing. The confirmatory laboratory test results and recommendation were: "Specific Gravity, creatine and PH low. Specimen dilute - suggest recollection." The Medical Review Officer concurred with the confirmatory laboratory test results and recommendation.

A urine specimen recollection was then performed. The recollected specimen was again sent to an approved confirmatory laboratory for testing. The confirmatory laboratory test results of the recollected specimen were: "Specimen adulterated - PH too low." The determination made by the Medical Review Officer, following review of the confirmatory laboratory test results of the recollected specimen, was: "Refusal to Test - Test not Performed." The employee's unescorted access authorization was revoked. The employee is no longer employed by PPL Susquehanna LLC.

Documentation of Isolated FFD Occurrence Licensee Employee - Not in FFD Random Pool PPL Susquehanna Security deactivated an employee's security badge as directed by senior management. The employee carried a prohibited item inside a backpack into the protected area gatehouse. Security detected the item in the x-ray machine. Site Access Services was notified by Security that the employee's security badge was deactivated.

Site Access Services removed the employee's access authorization and removed the employee from the FFD random pool.

The employee's security badge was subsequently reinstated by Security. However, Security did not notify Site Access Services that the employee's security badge was reinstated. The reinstated employee was absent from the FFD random pool for 13 days following the employee's reinstatement. During the 13 days the employee was inappropriately absent from the random pool, the employee was observed under the CBO program. This event has been investigated and dispositioned in the PPL Susquehanna deficiency management program. Security has revised their procedure to require that Site Access Services be contacted when a security badge is reinstated. The NRC was notified about this event, and requested that the event be documented in the semi-annual FFD report.
