Dear Mr. Agnes:

The attachments to this letter are Emergency Preparedness Materials developed recently by my staff based on our understanding of perceived weaknesses in offsite planning. He believe this information resolves the major concerns being addressed by the Federal Emergency Management Agency in their self-initiated review: Beach Population and Sheltering, Mobility Impaired and Transportation Dependent Individuals, Special Facilities Planning and a Northern Reception Center.

  • He recommend that your agency send this information directly to FEMA for consideration'during their self-1nitiated review. Prompt review of this information should preclude an unnecessary negative finding concerning offsite emergency planning for the protection of the health and safety of the general public around the Pilgrim Station, and in turn avoid creating unnecessary*

public concern.

We also recommend that the Massachusetts Civil Defense Agency use this information to updhte the State's Emergency Plan and Procedures as well as those of the five towns within the ten mile Pilgrim Emergency Planning Zone.

LICENSING ACTIONS Opened in August 1907 - 2 Closed in August 1987 - 4 Outstanding 08/31/87 - 44 SIGNIFICANT ISSUES AND EVENTS Significant Activities During August 1987 On August 21, 1987, Dr. Murley signed an Interim Director's Decision in response to the July 15, 1986 Petition regarding Pilgrim filed by Massachusetts State Senator William B. Golden and other:. Petitioners' requests on containment and emergency planning were denied. A decision on the management issue is deferred. The Decision also stated that the NRC will require Pilgrim to remain shut down until the management and emergency planning issues are dealt with to the satisfaction of the NRC.

On August 21, 1987 the staff issued the initial assessment of the Pilgrim Safety Enhancement Program. This program, including several significant changes to the Mark I containment (e.g. direct torus vent to plant stack) was submitted to the staff for infonnation July 8,1987. In its response, the staff has informed BECo that we are not prepared to endorse the use of the Direct Torus Vent System (DTVS) at this time. Additionally, the staff stated that installation of the DTVS cannot be made under the provisions of 10 CFR 50.59 and requested BECo provide answers to a number of questions regarding this system. Most of the other proposed modifications were found acceptable by the staff and can be implemented under the provisions of 10 CFR 50.59.

On August 20, 1987 the licensee reported that a portion of RHR piping on the inlet side of the RHR heat exchanger had failed visual inspections due to heavily pitted conditions on the external surface of the pipe. This is in a heavily contaminated area where there has been valve packing leaks in the past. It is believed that, due to poor housekeeping / maintenance practices, leakage has resulted in the presence of moisture between the RHR pipe insulation and the pipe itself. The utility is evaluating the significance of the pitting and the corrective action to be taken.

- 09/09/87 - Enforcement Conference in Region I to discuss noncompliance issues involving plant physical security.

- 09/24/87.- BEco to meet with !! headquarters Management (1-4 p.m.) to discuss ststus, emergency preparedness, ma,for plant modifications, and restart plans.

Items for Management Attention None Status of Items Previously Identified for Management Attention G. Requa, Project tingineer from PDII-I, has been loaned to the Pilgrim Project team for 6-8 weeks to assist with the licensing workload.

Projected loss of Project Engineer (currently assigned full time to Pilgrim) in late September may leave PDI-3 over-extended on Pilgrim Tasks.

g )h j f 8 A mp xrpM Pilgrim-Activities Update-8/5/87 On July 30, 1987 BECo submitted their Restart Plan, in accordance with conunitments to Region I. Region I will have the lead in the Plan's review and further revisions to the Plan are expected in the next 1-2 months. BECo continues to forecast rea 'iness to restart by September 30, 1987. Review assignments will be made by Region I and NRR. Preliminary review comments will be communicated to BECo at an onsite meeting on August 18, 1987.

The proposed 10 CFR 2.206 response to Senator Golden's Petition of July 15, 1986 is back to OGC for review after technical staff rewrite in response to CGC coments. Additional technical work on the Mark I containment portion is expected.

Review of the Pilgrim Safety Enhancement Program (SEP), received July 8,1987 is continuing. An initial assessment team visited BEco engineering offices July 22, 1987 and met with licensee representatives in Bethesda on August 4, 1987 as part of this review. Initial staff consensus is that most of the BECo l preposed changes can be implemented under the provisions of 10 CFR 50.59. On August 4,1987 the staff advised BECo that the Direct Torus Vent Modification -

would require licensing action and that we were not prepared to endorse installation of this modification at this time.

Region I is conducting a team inspection of modifications activity during the weeks of August 3 and August 10, 1987.

Preliminary results of a recent security inspection suggest escalated enforcement action may be required. Findings involve guard force and protected area deficier.cies.

Staff contact with FEMA indicates that the offsite emergency preparedness evaluation may be issued by the end of this week. Negative findings are expected.

LICENSING ACTIONS Opened in July ~1987 - 3 Closed in July 1987 - 0 Outstanding 07/31/87 - 46 SIGNIFICANT ISSUES AND EVENTS Significant Activities During July 1987 The NRR Project Manager and a multi-disciplined team visited Boston Edison Company (BECo) July 22, 1987 to make an initial assessment of the Pilgrim Satety Enhancement Program (SEP). This program, including several significant changes to the Mark I containment (e.g. direct torus vent to plant stack) was submitted to the staff for infonnation July 8,1987 The submittal describes hardware changes that BEco has voluntarily elected to implement at Pilgrim. BEco states in their submittal that none of the physical plant

-changes increases the probability or consequences of a design basis accident and that all of the changes will result in a reduction in the frequency. of core melt scenarios or an improvement in +he performance of the containment respase. BECo has advised the PM that all of the changes could be implemented under the pre /isions of 10 CFR 50.59. BEco has also advised the

. staff that they intend to implement these changes prior to restart of the Pilgrim facility (estimated by BECo to be in late September). Review is continuing and the resulti of the initial assessment will be available in the near future.

In early July, Region I, after discussion with ORPI/II, created a Restart Assessment Panel (chaired by the Deputy Director of the Division of Reactor Projects of Region I and including B. Boger as our senior member). In addition to coordinating regional activities involvincf Pilgrim, it was envisioned that this panel would coordinate various NRR activities where there was regional interface or scheduling matters of importance to BEco, and be a focal point for arranging NRR participating in major inspections. This panel would provide a focus for developing the staff's position regarding restart, and would July 23, 1987. incorporate NRR views and input. The Panel met most recently on subsequently that same day, met with Mr. Bird and senior members o nuclear staff.

On July 30, 1987 BECo submitted their Restart Plan in accordance with their i

comitments to Region I. Region I has the lead for this review and revisions to the Plan are expected in the next 1-2 months.

- Existing projects staff continues to be over-extended to support Pilgrim activity. Certain actions have not been completed (several Sholly notices, completion of licensing actions, follow-up on technical issues, and routine items due to insufficient resources).

Status of Items previously Identified for Manaaement Attention Much of the staff work on the response to the 2.206 Petition of July 15, 1986 from Senator Golden is complete. On August 4,1987 the package was returned to OGC for concurrence.
