Auxiliary Feedwater Pumps Numbers 12 and 13 6.

Description of Concern:

The overcurrent protective relays are set at cap 4.0 amp to trip at 107 percent of motor full load current for both auxiliary feedwater pumps. The 107 percent r'elay setting was verified by field inspection.

Significance of Concern:

If the auxiliary feedwater pumps are required to operate at their normal minimum voltage rating, which is ninety percent of the name plate rating the motors will be tripped by their overcurrent protective relays.

PG&E Memorandum dated Feb. 24, 1982 from E. H. Gove to Wallace Vahlstro-requests the tap settings on the Auxiliary Feedwater Pumps 12 and 13 overcurrent relays be changed from 4.0 amps to 4.5 amps, which would be 120 percent of motor full load rating.

Description of Concern:

The PG&E response to SWEC Document Request (140 stated FCV 37, FCV 38, and FCV 95 are designed to o. pen and close against a maximum differential pressure of 805 PSI per Purchase' Order 4R-8729B. These valves could be required to operate against a differential pressure in excess of 1100 PSI. As an example FCV 95 must open during conditions where steam generator safety valves are lifting. FCV 37 or FCV 38 could be required to close in the event of a steam line break with steam generator safety valves lifting.

Significance of Cone.arn:

The valve 6perators to FCV 37, FCV 38 or FCV 95 may not function under conditions where differe-e.ial pressure exceeds 805 PSI.

8.

Recornnendation :

PC&E should evaluate the operators ability to position the valves against the calculated maximum differential pressure against which the valves must function. Supporting documentation should be sent to SWEC for independent verification.

N As a result of meetings in October and December of 1982, BfL has identified various information needs.

The information request from the October meeting has been formally identified in the fiRC letter dated Decerter 1,1982 (Attachment 1).

The information request from the December meeting has been provided informally to the Project (Attachment 2) but we are still awaiting the formal request from flRC.

In addition, on January 13, the fiRC and Brit requested additional information to clarify information previously submitted (Attachment 3).

In response to tilese requests, the Project has provided structural and piping analyses packages to Blit in submittals dated December 10 and 17,1982 and January 4,1983, as well as informtion provided during the December meeting, liost of the requested information has been included in these packages. The information which rcmains to be submitted is listed below:

1.

Structural drawings showing modifications to the annulus due to DE and DDE.

2.

PG&E analysis results for piping problems 8-116, 8-117, and 8-118.

3.

DCP's review of the drawing list on pages 16-17 of i(VREG/CR-2834 to insure they are the latest revisions; if not, up-to-date information to be provided.

4.

How are Class 1, 2, and 3 evaluations implemented in itE 101 and liestd>Ti computer codes? fleed clarifications for input and output statements used in both codes.

A list of coordinates for all nodes in piping problems 6-4 and 6-7.

3.

A sumary of nodal connectivity for piping problems 6-4 and 6-7.

(All three items would be satisfied if full size drawings for 2

these problems were provided including the node locations.)

A printout of steam generator weight distribution data. A copy was provided that is not completely legible.

The status of the above remaining 11 items is indicated on Attachment 4.

The Project has committed to update, as appropriate, the information which has been provided to BNL.

It is also expected that the Project will need to hold further discussions with BNL to confirm that the structural and piping information is consistent with eac_h other. Accordingly, Licensing is continuing to request, through the NRC, that BNL conduct discussions with the Project ~ through either meetings or telephone. conversations. Licensing will apprise the Project of any developments.

That decision con-tains our findings of fact and conclusion of law on the con-tested security plan issues in the operating license pro-ceeding for Pacific Gas and' Electric Company's two-unit Diablo Canyon nuclear facility.

Because ALAB-653 RESTRICTED contains numerous details of the facility security plan, it must be treated as protected information and may not be publicly disclosed.

Our nltimate conclusion is not so re-stricted, however, and provides a public record of our action on-the security plan issues.

Based on our review of the entire record on the security plan issues, and subject to the conditions and exceptions noted in ALAB-653 RESTRICTED, we conclude that the applicant's security plan conforms to all applicable provisions of the

1 Discussion Mrs George Maneatis, PG&E Senior Vice President for Facilities Development, states that PG&E has engaged Bechtel for the completion of the Diablo Canyon Project 1

with management personnel responsible for the overall direction and with profes-a j

sional staff to supplement the PG&E organization. The resulting Diablo Canyon Project Organization (Enclosure 4) is a single, totally integrated organization of Bechtel and PJ.&E personnel. Maneatis stated that other engineering orge,nizati.

4 had been considered. Bechtel was selected on the bases of availability, manageme and technical. support capability, and geographical location, among other factors.

j Maneatis stated that Bechtel has perfomed engineering functions for PG&E on othes projects in the past but is not retained by PG&E on a continuous basis.

Mr. Howard Friend of Bechtel is the Project Completion Manager and will report directly to Maneatis. Friend stated that the objective of the organization is to fulfill all requirements for reinstating the low power license of Unit 1 and for meeting full power licence requirements for both units..To this end a-team of senior PG&E and Bechtel managers has been fomed. On September 10, 1982 R. F. Reedy Inc., the IDVP organizaticr cerforming the QA audits within the program, conducted an exit meeting on'Pnase II QA audits with representatives of Pacific Gas and Electric Company (PG&E).

Mr. T. W. Bishop, a member of the NRC Region V staff, attended the meeting and prepared a summary of the meeting including related comments (memo-randum from R. H. Engelken, Regional Administrator, to H. R. Denton, Director, Office of Nuclear Reactor Regulation, dated September 15,1982).

Although the NRC has not approved yet the Phase II plan for the IDVP, we believe that the Region V comments on Phase II activities already in progress,.such as the QA audits, are appropriate.

We therefore are providing you with the above mentioned memorandum for your consideration.

We request that you consider the co.mments in your evaluation of the Reedy findings, particularly with respect to _the limited scope of the audit as compared to the scope as specified in the NRR letter of November 19, 1981.

It is our understanding that the Reedy report (s) on the QA ~ audits will not be issued until mid-October. We request that you provide a status of the QA audits in your next semi-monthly report.

This work included the exploration, exploitation and development of energy scurces, and the siting of energy facilities.

General Dynamics Corp./ Electric Boat Division, Groton, CT, (1963 to 1977):

(a) Deputy Program Manager (1975-1977) - responsible for the construction program of nuclear reactor plants in submarines.

Directed engineering, procurement, manuf acturing, installation, inspection.and testing efforts to ensure that the nuclear plant met the contract specifications and was delivered on schedule.

(b) Chief of Mechanical Systems and Components (1973-1975) - directed the efforts of engineers in the development of system design descriptions and fluid flow diagrams (P& ids) of various reactor plant support systems of a land based prototype nuclear reactor plant. Work included plant and equipment arrangements, specification and procurement of ccmponents, instrumentation and controls, piping, schedules, estimates, and contract administration.

Also included was the prepa' ration of the PSAR and FSAR.

Technical requirements were to the ASME B&PVC and the various NRC regulatory guides to the maximum extent possible and as supplemented by Naval Nuclear Design Standards and Codes.

(c)

Chief of Reactor Systems (1969-1973) - directed the engineering and design efforts.for the development of a nuclear submarine reactor plant.

Work included the preparation of fluid flow diagrams, construction drawings, power plant operating manuals, test procedures, equipment technical manuals, and specification and procurement of equipment.

(d)

Senior Nuclear Project Engineer (1964-1969) - developed fluid flow diagrams and system design descriptions. Directed design (draf ting) efforts in the preparation of construction drawings of varicus reacter sysums.

(e) Nuclear Test Engineer (1963-1964) - performed preliminary operation and performance testing of reactor plant systems. Prepared test procedures and schedules. Determined the acceptability of data. Participated in dock and sea trials.

Dear Dr. Cooper:

Enclosed please find a copy of the transcript of a meeting held on January 6,1983, between members of the NRC Staff and an individual concerning alleged safety deficiencies at the Diablo Canyon Project.

In particular, the stated concerns relate to:

a.

design aspects of the component cooling water system, including its seismic classification, single failure considerations and its heat removal capabilities, and b.

various other considerations including I&C design /

classifications approaches, design approaches used for seismic Class I and Class II interfaces, and the seismic design of diesel, generator intakes and exhausts.

Both the substantive and procedural concerns raised should be factored into the Independent Design Verification Program as appropriate.

The allegations have potential bearing on the currently ongoing verification efforts of both the utility and the IDVP. Accordingly, notification of these allegations was transmitted to the Comission by Board Notification No. 83-03 on January 7,1983 (copy attached).

The alleger has expressly requested and was assured of anonymity to the extent permitted by law. Consequently, as you have agreed, the enclosed transcript is to be treated confidentially. No copies of the transcript should be made and access to the single copy provided should be restricted (and duly documented) to the smallest number of individuals possible consistent with your need to evaluate its contents in conjunction with the IDVP. Appropriate measures to control dissemination of the information should be employed to assure preservation of the anonymity of the alleger.
