SUMMARY

The issues identified in this element refer to a possible programmatic deficiency in the engineering corrective action process. The issues involve a concern that practices within engineering require an approval cycle for documentation of conditions adverse to quality (CAQ), and the approval cycle hinders the reporting of conditions adverse to quality by employees.
The programmatic impact of this concern being valid would be the potential need for modifications to the procedures and practices governing the CAQ process. Such modifications would be oriented towards improving the effectiveness and freedom of the corrective action system.
Investigation to date indicates that engineering procedure changes have been accomplished which address the issues identified in this element.
Specifically NEP-9.1, " Corrective Action" (Rev. O, July 1,1986), now provides measures enabling the originator of a CAQ to escalate a concern to higher
-management - in the event of disagreement between the originator and supervisor on the validity of the concern. The previous engineering procedure (OEP-17) did not provide for such recourse. In addition to the modification to the engineering procedures, the existence of the TVA Employee Concerns
- g.9 Program provides a method available to the employee to circumvent undue j '
restrictions in the CAQ process.
Therefore resolution of this programmatic item is not considered to be a requirement for plant restart.
