UNREVIEWED SAFETY QUESTION As defined in 10CFR50 and 10CFR72 applies to changes, tests or experiments.

10CFR50.59 (a) (2) and 10CFR72.48 (a) (2) state:

"A proposed change, test, or experiment SHALL be deemed to involve an unreviewed safety question (i) if the probability of occurrence or the consequences of an accident or malfunction of equipment important to safety previously evaluated in the safety analysis report may be increased; or (ii) if a possibility for an accident or malfunction of a different type than any evaluated previously in the safety analysis report may be created; or (iii) if the margin of safety as defined in the basis for any technical specification is reduced."

5.0 RESPONSIBILITIES 5.1 The General Superintendent Engineering (plant & NGS) SHALL be responsible for assuring safety evaluations are completed when necessary.

5.2 The Preparer of the Project Description / Safety Assessment (PD/SA) SHALL be responsible for preparing the PD/SA in accordance with this instruction and SAWI 3.3.2, Safety Evaluations.

5.3 The Reviewer of the PD/SA SHALL be responsible for ensuring the technical adequacy of the PD/SA and its conformance to the requirements of this instruction.

5.4 The Operations Committee SHALL be responsible for reviewing the PD/SA for impact on plant safety.

5.5 The Design Engineering Coordinator (DEC) SHALL maintain an index and computer data base of completed PD/SAs.

SAFETY ASSESSMENT 5AWI 6.1.4 REv: o l 6.0 REQUIREMENTS AND RECOAIMENDATIONS l 6.1 General Requirements & Rocommendations 6.1.1 This AWI contains requirements for preparation and content of Project Descriptions and Safety Assessments and their use in proposed plant design changes.

6.1.2 5AWI 3.3.2, Safety Evaluation SHALL be used when it is determined to be necessary in the Project Description / Safety Assessment. .

6.1.3 The Project Description SHALL include justification if the design change is performed to less than original specifications or design bases.

6.1.4 A Project Description SHALL comprise / coordinate design criteria as design input i for projects. Large scale projects may accumulate numerous additional inputs as detailed design proceeds. The Project Engineer determines the manner of tracking design inputs not indicated in the Project Description,5AWI 6.1.1, Design input.

6.1.5 If a Preliminary Project Description is desired by management, it should follow

! the same format as the Project Description to clearly define the plant's needs and technical requirements for the project.

a. When the Preliminary Project Description is prepared, the Project Engineer should:
1. Obtain concurrence from the Solution Team that the Preliminary Project Description is ready for initial review.
2. Distribute copies of the unsigned Preilminary Project Description to the Solution Team members,
b. The preliminary Project Description may be revised as necessary to adequately describe project development during the planning phase of the design change. The Preliminary Project Description should be transformed into the final Project Description for inclusion in the Design Change Package.

6.1.6. The Project Engineer should resolve or incorporate comments of the Solution Team into the project's PD/SA.

SAFETY ASSESSMENT REV: g G.2 Prolect Description content 6.2.1 The preparer SHALL use PINGP 1214 or equivalent to compile the Project Description / Safety Assessment.

6.2.2 The preparer SHALL transfer the electronic file via E-mail to the DEC for i electronic storage of completed / reviewed PD/SA.

6.2.3 The PD/SA of the first part of a multiple part design change SHALL contain a brief description of each of the parts.

! 6.2.4 The Project Description SHALL include the following to the extent that they are applicable to the plant design change or part of a plant design change covered by the description:

a. Title Page l

Use the same title as the IPC approved title, indicate the design change number and the proper part and revision number.

b. Jntroduction .

A summary of the scope, purpose and design objectives for the entire integrated project. Indicate quality classifications such as safety related, fire protection related, or security related. If only portions of a project require special quality requirements, then specifically state the boundaries of the OA classifications.

l c. Summarv Summarize the significant points of description and Safety Assessment that establish the Safety Assessment conclusion. The summary is written such that the test may be used for completing the USAR/ISFSI SAR change input.

d. Scope A detailed and concise definition of the specific work scope covered by the i

design change. Consider mechanical, civil, structural, nuclear, electrical, operational and I&C attributes in the description. This is particularly i

important when a plant design change is performed in parts. Define the scope of the project by specifying impacted structures, systems, or g

components.

e. Descriotion A description of the plant design change provides sufficient detail to enable staff and a review committees (some of which do not have detailed ,

knowledge of the plant) to understand what structures, systems and components are to be added or modified. The description includes or refers l to electrical schematics, logic diagrams, P&lDs, one-line diagrams, general structural layout drawings, sketches, etc., as needed. Also, discuss the design basis possible failure modes considered for the design change, accident review. Refer to SAWI 3.3.2, Safety Evaluations, if a Safety  ;

Assessment is required. Consider mechenical, civil, structural, nuclear, electrical, operational and l&C attributes in the description.

1. Provide important functional design parameters, such as pressures, flows, temperatures, trip setpoints, alarm settings, etc.
2. Describe design features, operating functions, and interlocks. The

, reader should be able to understand how the design performs its function.

3. Specify the plant operating problems or needs that are to be addressed, and the regulatory requirements or commitments that are to be satisfied by the project.
f. Oraanizations involves Description of the groups and organizations associated with the plant design change and their scope of involvement. This includes specifying design by consultants, contractors, or other outside design organizations.
1. For plant design changes subdivided into several parts and assigned to different organizations, preparation of documented division of responsibilities is considered.
2. The Project Engineer SHALL notify other organizations that are required to approve, or be notified of the plant design change. This notification should include organizations to which commitments impacted by the design change have been made or are affected. This notification should consist of:

a) A description of the design change being made.

g. Engineerina Considerations & Desian Inouts.

A description of engineering considerations with principal benefits is included. Information comprising design inputs is defined here until otherwise dispositioned by the Project Engineer for design control of complex design changes.

1. Specify the technical requirements of the project and special considerations that are to be addressed in the project. The Design Change input Applicability checklist (PINGP 1213) SHALL be utilized to identify engineering considerations.
2. If a Safety Assessment is required, the following 5AWI 3.3.2 content information is also required.

a) A discussion of the effects of the design change.

b) A statement concerning changes to Technical Specs.

3. Identify how operational holds will be applied in order to ensure adequate holds are placed on the plant to prevent the plant from being placed in a condition requiring the system or component being modified to be operable prior to the modified system or component being turned over for operations. Refer to 5AWI 6.1.5, Design Change implementation Plan, for operational hold codes.
h. Materials. Codes & S_landards
1. Specify the codes and standards to be utilized with appropriate edition / addenda, revisions, or date. These SHALL include codes and standards such as ASME, IEEE, ANSI, AWI, AISC, or NFPA. It is not intended that manufacturing or product standards such as NEMA, UL, or ASTM be included. However, regulatory documents such as 10CFR, 49CFR, NRC Reg Guides, NSP Commitments, NUREGs, and NRC g

Correspondences should be quoted or referenced as applicable.

2. Discuss the most important specification; i.e., codes, standards, l- materials, components, specifications, etc.
i. Construction / Plant Interface Considerations Discuss potential impacts that construction may have upon the plant from an operations and safety viewpoint. Define construction commitments which will eliminate or mitigate these impacts.

(3) 6.3 Review and Approval 6.3.1 The originator signs and dates the Project Description / Safety Assessment.

6.3.2 Project Descriptions / Safety Assessments are reviewed for accuracy by an individual other than the preparer who is knowledgeable of the design change.

Project Descriptions SHALL be signed and dated by the reviewer. This review is not required to serve as design verification (elective) since design verification is normally done on individual design documents in preparation of detailed design output for construction.

6.4 Revisions 6.4.1 Changes to items previously reviewed / approved by the Operations Committee may be approved by the Gen Supt. Engineering (plant) if there is no change in acope or intent.
