.f PREFACE This report summarizes the preservice and inservice inspections performed at Maine Yankee Atomic Power Plant between July 1, 1984 and November 17, 1985 that fall within the scope of Section XI of the ASME Boiler and Pressure Vessel Code and the Maine' Yankee Inservice Inspection Program. The end of the first inspection period of the second ten year inspection interval is December 29, 1985. The Authorized Nuclear Inservice Inspector on-site to witness the inspections was Mr. Harry Evans of Hartford Steam Boiler Inspection and Insurance Company.

CGIEGQBY CQtOIIION B-G-2. One Steam Generator Primary Manway Stud was rejected visually due to excessive galling in the shaft area. This galling was a result of improper extraction.

B-P One 1" socket meld was found to be leaking during the Reactor Coolant System Leak Test. The weld was in the Reactor Coolant Pump Sealmater Injection System.

< 20% 2 22 17 The tube wall degradation was detected in 3 key areas: the center vertical support strap, the region associated with a sludge pile, and the steam blanketed region postulated to exist at U-bends in rows 5 thru 8. The defect indications at the vertical straps and tight U-bends are characteristic of a corrosion phenomenon. This is consistant with the suspected presence of dry-boiling and a resultant concentration of causative contaminants in the effective regions. Pitting type indications were observed immediately above, at the top, and below a classical sludge pile surface contour associated with a PWR steam generator.

No recordable defects were found during the remaining examinations.

B-P The leaking weld found during the RCS Leak Test was isolated, ground out in area of defect, and rewelded. A liquid penetrant exam was performed after grinding to ensure that the entire defect was removed. A final liquid penetrant exam and leak test was performed to verify the integrity of the weld.

B-G In Steam Generator 1, the 3 tubes which were found to have indications greater than 40% through wall were plugged.

In Steam Generator 2, the 19 tubes which were found to have indications greater than 40% through wall plus 3 others were plugged.
