2.2 SCOPE l This procedure applies to personnel perfonning nondestruc-ttve examinations (NDE) or providing tecnnical support of ,

Doctmented testimony by the Site NDE Level III to the acceptability of the qualifications of personnel.

2.3.4 Levels of Certification There are three levels of certification in each method or discipline:

a. Level I
b. Level II
c. Level III 2.4 GENERAL REQUIREENTS 2.4.1 Certi fication Personnel performing NDE shall be trained, qualified and certified in accordance with this procedure.

NDE personnel currently certified at CPSES are considered certified until the expiration of their current certification.

Recertification shall be in accordance with this instruction.

Level III personnel certified to equivalent programs may be certified by the TUGC0 QA Supenisor without re. examination, providing:

a. The TUGC0 QA Supenisor has reviewed and accepted the program under which the individual was certified as being equivalent to this program;
b. The individuals previous certification was by examination; and
c. The certification and supporting documents are placed in the employees certification file.

The effective period of certification for all certifications issued in accordance with this instruction shall be three years. Certification of qualification shall be doctmented on Attachment 1. Certifications may be revoked / suspended by the Site NDE Level III when reason exists to question the individual's ability to perform NDE satis factorily.

Education, training and experience requirements for each level of certification are prescribed in this instruction. Personnel may be qualified and certified to perfonn examinations of limited scope at each respective level of certification.

The number of hours of training and the amount of work time experience requi red for inspection personnel who perfonn only one operation of an examination method that consists of more than one operation, or perfonn examinations of limited scope may be less than that specified, as approved by the Site NDE Level III.

Personnel holding limited certifications perfonn only those operations allowed within the restricted scope of the certi fication. Wen perfonnance of an examination requires operations beyond the scope of the limited certification, personnel qualified to perfonn those operations shall be utilized.

Certification limitations are documented on Attachment 1.

2.4.3 Prior Experience or Training

Previous training and/or work experience may be used to partially or fully satisfy the qualification requirements prescribed in this procedure, when documented and approved by the Site NDE Level III.

2.4.4 Examinations Candidates for certification are given examinations covering the general, specific and practical aspects of the method.

Examinations are prepared and approved by the Site NDE Level III and administered by the Site NDE Level III or his designee.

A minimum emnposite grade of 80 percent is requi red for qualification of Level I and Level !! personnel. In ad- I dition, individual passing grades for the general, specific and practical examinations are 70 percent or greater. Test objects used in the practical examination contain known indications, and at least 90 percent of the known indications mst be found during the practical examination.

For Level III, a composite grade of 80 percent is required, ,

and individual passing grades for the basic, method and specific are 80 percent or greater.

Failure by a candidate to meet the above required grades will result in dental of certification.

2.4.6 Grading Factor A percentile weight factor is applied to the raw scores of the various examinations, i.e., general, speci ff t. and l practical. These factors are as follows:  !

Personnel do fail two consecutive examinations for the same certification, are not eligible for certification in that method. ,

2.4.8 Recertification NDE personnel are recertified at least once every three years in accordance with one of the following criteria:  ;

a. Re-examination in the practical aspects of the method using a grade factor of 1.0; or
b. Re-examination in the general, specific and practical aspects of the method v11ng the grade factors listed in 2.4.6.

QC Supervisors shall notify the Site NOE Level 111 of NDE personnel under their supervision having interrupted service exceeding six mnths. NOE personnel having interrupted servica exceeding six months shall be recerti fi ed as described in 2.4.8 a.

2.4.9 Termination and Reinstatement

. All certifications are automatically revoked when pere 'nnel terminate mployment at CPSES.

The certification of s ployees terminated and re-hi red within six months may be reinstated without additional testirq if approved by the Site NDE Level !!!, or by the TUGC0 QA Supervisor for Level III personnel.

New splayees may be recerti fied to a former level of certification, granted by a previous uployer based on an examination in accordance with this instruction provided all of the following conditions are met to the satisfaction

, of the Site NDE Level III:

a. The nov uployee has proof of prior certification and such documentation is placed in his certification file;
b. The new employee's neber of hours of training and work time experience are veri fi ed , and meet the requirments of Attachment 2;
c. The new employee was working in the capacity to which he has been certified within six months of his termination; and
d. The not uployee is being recertified within six months of his termination.

requirements of this procedure and be certified as either Level I, II, or III.

3.2 EDUCATION, TRAINING AND EXPERIENCE Personnel considered for certification are required to have suf ficient education, training and experience to ensu re innderstanding of the principles and procedures of those areas of responsibility in which they are being considered for certification. Education and experience requirements are prescribed on Attachment 2.

3.3 IN00CTRINATION The following are minimum indoctrination and training requirements for inspection personnel . Indoctrination requirenents shall nomally be satisfied within the first sixty days of employment, and shall be documented on Attachment 3.

Personnel satisfying documented technical training require-ments may be assigned to inspection or testing functions

seminars or other appropriate means. An oral checkout covering ANSI N45.2 and the 18 Critaria will be given by the responsible supervi sor who shal l , upon satisfactory completion, so note by signing the General Training Outline (Figure 1).

b. Industry Standards Inspection personnel should become familiar wi th prepared standards ( ANSI, IEEE, ASME, etc.) applicable to assigned duties. The method of presentation may be threugh study, fomal seminars, or other appropriate means. Supplemental exposure is acqui red by daily association with these standards. The familiarization is a continuing process and as such requires no fomal chec kot.t.

understanding of the basic requirements as they relate to job duties and responsiblities, the individual shall so note by signing the Training Outline. Fomal or infonnal training sessions shall emphasize the need for prampt repo rting of product deficiencies or failures to the TUGC0 Site QA Supervisor.

3.4 RESPONSIBILITY 3.4.1 Trainee In the process of being qualified and certified to at least Level I, an individual is considered a Trainee. A Trainee works along with a certified individual and does not conduct independently any test, interpret any results of a test, or write a report of test results.

3.4.2 Level I A Level I individual shall be qualified to properly perfonn specific calibrations, specific tests and specific evalua-tions according to written instructions and to record the results. He shall receive the necessary guidance or supervi sion fran a certified Level II or III individual .

3.4.3 Levi!1 II

, A Level !! individual shall be quali fied to set up and calibrete equipment and to interpret and evaluate results with respect to applicable codes, standa rds and speci fications. He shall be familiar with the scope and limitations of the method and shall exerci se assigned responsibility for on-the-job training and guidance of Trainees and Level I personnel . He shall be able to prepare written instructions and to organize and report nondestructive testing investigations.

3.4.4 Level III An HDE Level III shall be responsible for establishing i techniques; interpreting codes, standards and specifications; and designating the particular test method and technioue to be used. He shall be responsible for the complete operation he is qualified for and assigned to, and shall be capable of evaluating results in tenns of existing codes, standards and speci fications. He shall have sufficient practical background in applicable techniques to assist the design engineer in establishing acceptance criteria dere none are l

personnel for certification.

3.5 LEVEL I AND II EXAMINATIONS Each candidate for Level I and II certification is given a three-part examination covering the general, specific and practical aspects of examination.

All three parts of the examination are administered by the Site NDE Level III or his designee.

The written general and specific ' examinations are administered without access to reference material (closed book) except for reference material specifically approved and deemed necessary by the Site NDE level III.

3.5.1 General Examination General examinations shall cover the tessic test principles relative to the applicable method.

Specific examinations shall cover the equipment, operating procedures, examination techniques, project specifications or cooes and acceptance criteria in the project examination procedures or instructions.

i 3.5.3 Practical Examination l

Proficiency shall be demonstrated in selecting and perfonning the applicable nondestructive examinations and evaluating the results obtained on one or more samples containing known indications, approved by the Site NDE Level III.

At least 90 percent of the known indications mst be detected.

At least ten di fferent checkpoints requi ring an understanding of test variables and QA Procedure or Instruction procedural requirments shall be included in the practical examination.

The description of the specimen, the examination procedure including checkpoints, and results of the examinatior shall

, be docuented.

of limited scope may be less than that specified when approved by the Site NDE Level III, (eg. LITDT.) l 3.6 LEVEL III CERTIFICATION REQUIREMENTS r I

The TUGC0 QA Supervisor is responsible for examining and

, certifying the Site N9E Level III in accordance with this procedure.

1 3.6.1 Level III Education and Experience Requirements

Individuals being considered for NDE Level III in any one method must meet one of the following educational and/cr experience requirements:

a. Graduate of a four-year accredi ted engineering cr I l

science college or university with a degree in I Engineering or Science plus one year's experience in an ass!gnment comparable to that of a Level II in the applicable test method;

b. Completion with a passing grade of at least two years of engineering or science study at an accredited university, college or technical school plus two years i

experience as a certified Level II in the the applicable test method; or j

3.6.2 Level III Examinations The Level III examination consists of the following:

a. Basic Examination (Required only once when more than one method of examination is taken).

Twenty (20) questions relating to understanding the SNT-TC-1A document.

Fifteen (15) questions relative to applicable materials, f abrications, and product technology.

Fifteen (15) questions dich are selected from or are similar to published Level II questions for other appropriate NOT methods.

b. Method Examination (for each method).

1 Thirty questions relating to fundarwentals and pri nciples, which are selected fram or are similar to publi shed ASNT Level III questions for each method, and Fi fteen (15) questions relating to application i

and estalishement of techniques and procedures which are selected fram or are similar to the published ASNT Level III questions for each method, and Twenty (20) questions relating to capabili ty for interpreting codes, standards, and specifications relating to the method.

c. Sr.ecific Examination (for each method).

Twenty (20) questions relating to speci fications, equipment, techniques, and procedures applicable to the employer's product (s) and methods aployed , .and to the administration of the employer's written practice.

! l 3.7 VISION REQUIREMENTS 3.7.1 Near 01 stance Visual Acuity Each candidate for certification in inspection cethods to 1 Level I, II or III shall be examined for near distance i vi sual acui ty and color perception on an annual ba si s.

The examination is given to assure natural or corrected near distance acuity in at least one eye such that the candidate is capable of reading a minimum of Jaeger Nunber 1 letters on a standard Jaeger Test, at a distance of not less than 12 inches (30.5 cm). The ability to perceive an ortho-rater minimum of 8 is also acceptable. Similar tests may be used if approved by the Site NDE Level III.

l 3.7.2 Color Perception Individuals shall be capable of .. stingui shing and differentiating contrast between ,,a i o rs , as evidenced by correctly identifying at least 10 out of the first 11 plates of the Ishihara Color Perception Eye Examination.

Personnel failing to correctly identify at least 10 of the first 11 plates shall be given a practical examination approved by the Site NDE Level !!! using colors dich the individual can expect to' encounter in hi s specific assignment. The results of the practical eye examination shall be documented on the eye examination fann.

The personnel training files of each certifled individual shall contain a copy of the completed eye examination fonn.

3.8 TRAINING PROGRAM Training programs for each NOE method are developed by cognizant Quality Engineers and approved by the Site NDE l Level III.

Training programs shall be presented by instructors designated by the Site NDE Level III, (Attachment 4) using lesson plans approved by the Site NDE Level III. Records of attendance at classroom training shall be documented on Training Records (Attachment 5) and maintained in the individual's personne.1 training and certification file.

3.9 CERTIFICATION RECORDS REQUIREMENTS 3.9.1 Certification Documentation Written certification is provided in accordance wi th Attachment 1. The following items shall be documented on the personnel certification record, as applicable:

a. Name and Social Security Number;
b. Level of certification and test method;
c. Highest level of education completed; i d. Training received;
e. Date of eye examination;
f. Total accredited work time experience; 1 g. Certification exam msnbers, date completed and scores;

. h. Percentile weight assigned to each examination;

i. Composite grade of all examinations;
j. Renarks or comments on above areas; i
k. Date of certification or recertification;
1. Date of certification expiration; and
m. Signature of certifying authority; Training and Certification Files Personnel training and certification files shall contain the following documentation, as applicable;
a. Certification Record (s) (Attachment 1);
b. A summary of Education and/cr work Experience (Attachment 6) developed from documented '.erification letters;

3.9.2 DOCUMENTATION FILING Doctanentation shall be mintained by the 3 Training Coordinator. Files shall not be removed from the controlled area without the concurrence of the QA Training Coardinator.

SUBCONTRACTED NDE PER56NNEL Subcontracted NDE personnel, including the NDE Level III are certified by the subcontractor in accordance with written procedures which comply with Reference 1-A and have been approved by the TUGC0 QA Supervisor.

The Subcontractor's personnel quali fications shall be reviewed by the TUGC0 QA Supervisor to verify compliance with the Subcontractors written practice. The Subcontractors NDE -

Level III shall be authorized by the TUGC0 QA Supervisor to act for TUGCO. Approval of the written procedure and personnel qualifications shall be documented by memo to the subcontracted agency by the TUGC0 QA Supervisor. Copies of these records shall be retained.
