I am a visual and NDE Welding Inspector. I do most of my 5 A.

6 inspection in the Auxiliary Building, but my duties as an inspector l'

7 carry me to all parts of the plant.

8 Q. SUMMARIZE YOUR EXPERIENCE AND QUALIFICATIONS, INCLUDING 9 OTHER NON-DUKE JOBS, EDUCATION, CERTIFICATIONS, AND 10 COMPANY SPONSORED COURSES AND TRAINING.

r i 11 A. I worked as learner in the Welding Department at Duke Power ad 12 there I was trained as a welder. I went through Duke's welding l 13 course and was certified and welded in the Reactor Building at 14 Catawba from 8-22-77 to 7-9-79. I also have a small welding shop l at my home and have 'been around welding for 20 years. On l 15 l 16 8-20-79, I completed my trauung as welding inspector and have 17 been certified as a visual inspector for 4 years. I recently was i

certified as an NDE inspector. I have been an inspector at 18 19 Catawba for about 4 years.

2 Q. ARE YOU FAMILIAR WITH WHAT IS COMMONLY REFERRED TO AS 3 THE WELDING INSPECTOR CONCERNS WHICH WERE EXPRESSED IN 4 LATE 1981/EARLY 1982? ,

5 A. Yes.

6 Q. WHAT IS YOUR UNDERSTANDING OF WHAT THESE CONCERNS .

7 WERE?

t 8 A. I am not entirely sure what others specific concerns were. My 9 concern is described in answer to another question.

l 10 Q. DID YOU EXPRESS ANY CONCERNS AS A WELDING INSPECTOR TO 11 'ANY OF THE TASK FORCES OR TO DUKE POWER MANAGEMENT?

12 A. Yes.

13 Q. TO WHOM DID YOU EXPRESS YOUR CONCERNS?

14 A. I talked to Task Force I and Gail Addis.

I 15 Q. WERE YOUR CONCERNS WRITTEN?

16 A. Yes.

17 Q. DESCRIBE YOUR CONCERNS, AND INDICATE WHO THEY WERE 18 SUBMITTED TO.

19 A. I addressed my concern to Bob Morgan. It was on an NCI that I 20 had written. It concerned the marking of a socket weld on 21 non-safety piping per CP 300 (a construction procedure). On a 22 random inspection I had discovered that the craftsman had failed to These welds l 23 put a scribe line on the welds they were working on. ,

24 were tacked up and some were welded up, so I wrote an NCI 25 because craft had not followed CP 300. This was a rush item and 26- the craft general foreman (welding) walked the NCI through. The 27 NCI was resolved by stating the welds were acceptable. The 28 resolution took into consideration among other things, the fact that

n 1 this was non-safety piping. While I agreed that this was not a

,' 2 safety concern, I still felt that my management had not backed me 3 up like they should have.

4 After a meeting with Bob Morgan and Charles Baldwin, I felt i

5 that they were real interested, in my problems and they were helpful 6 when they talked with me on this problem that I had.

7 Q. DID YOU FEEL FREE TO EXPRESS ALL OF YOUR CONCERNS?

8 A. Yes.

9 Q. DID YOU EXPRESS ALL OF YOUR CONCERNS?

10 A. Yes.

11 Q. HAVE YOU DISCUSSED YOUR CONCERNS WITH ANYONE ELSE?

12 A. No. .

13 Q. HAS ANYONE ELSE CONTACTED YOU AND ASKED YOU TO 14 DISCUSS YOUR CONCERNS?

16 Q. WERE YOUR CONCERNS INVESTIGATED BY THE TASK FORCES?

17 A. No.

18 Q. DID YOU ATTEND ANY MEETINGS WITH TASK FORCE AND/OR QA

20 CONCLUSIONS AND RECOMMENDATIONS WERE DISCUSSED?

21 A. I was interviewed by one of the Task Forces but don't recall which 22 one.

23 Q. WERE THERE ANY CHANGES MADE IN THE QA PROGRAM AFTER 24 THE WELDING INSPECTOR CONCERNS AND THE TASK FORCE 25 INVESTIGATION OF THESE~ CONCERNS?

26 A. Yes.

i 27 Q. DESCRIBE THE CHANGES OF WHICH YOU ARE AWARE IN THE QA 28 PROGRAM.

,s 1 A. There have been many changes such as more training in QA and I

- 2 Construction Procedures. Also the upper level supervision (such 3 as Larry Davison) seems to be more interested in problems that you 4 have on the job.

5 Q. DO YOU CURRENTLY HAVE ANY PROBLEMS WITH THE 6 FUNCTIONING OF THE QA PROGRdM WHICH HAVE NOT BEEN 7 RESOLVED?

8 A. No.

9 Q. DID THE EXPRESSION OF YOUR CONCERNS INDICATE YOUR 10 BELIEF THAT THERE WAS A BREAKDOWN IN THE QA PROGRAM 11 OR INDICATE THAT THE QA PROGRAM WAS NO LONGER 12 WORKING?

13 A. No. However, I believe that there may have been problems in 14 communications .

15 Q. DO YOU BELIEVE THAT THE CATAWBA PROJECT IS BEING 16 CONSTRUCTED SAFELY?

17 A. Yes.

18 Q. IN YOUR VIEW, HAS THE QA PROGRAM BEEN EFF3CTIVE WHILE 19 YOU HAVE WORKED AS AN INSPECTOR AT CATAWBA?

20 A. Yes.

21 Q. ARE YOU AWARE OF ANY DEFICIENCIES IN CONSTRUCTION OR IN

/ 22 THE QA PROGRAM WHICH WOULD CAUSE YOU TO QUESTION 23 WHETHER CATAWBA IS SAFELY BUILT?

24 A. No.

i 1 A. I know that Catswba is built as safe as it can be. Our QA Program 2 is one of the best and I know that Duke Power is concerned with 3 the safety of its plants.
