9 A. Welder - Fabricator, Gaston County Dye from 1975 - 1977.

10 GED Catawba Valley Tech. Welding Inspector A at McGuire from 11 1977-1980, and from 1980 to present, Welding Inspector A at 12 Catawba.

15 A. NDE magnetic particle testing and liquid penetrant testing 16 inspector.

1 Q. ARE YOU FAMIL1AR WITH WHAT IS COMMONLY REFERRED TO AS 2 THE WELDING INSPECTOR CONCERNS WHICH WERE EXPRESSED IN 3 LATEu 1981/EARLY 1982?

4 A. Yes.

5 Q. WHAT IS YOUR UNDERSTANDING OF WHAT THESE CONCERNS 6 WERE?

7 A. That there was a lack of communication between QA inspectori; and 8 upper management, and we were at times being overruled by 9 differing interpretations and investigations of upper supervision 10 without being told why.

11 Q. DID YOU EXPRESS ANY CONCERNS AS A WELDING INSPECTOR TO

12 ANY OF THE TASK FORCES OR TO DUKE POWER MANAGEMENT?

13 A. Yes. -

14 Q. TO WHOM DID YOU EXPRESS YOUR CONCERNS?

15 A. I expressed my concerns to Bob Morgan.

16 Q. WERE YOUR CONCERNS WRITTEN?

17 A. Yes.

20 A. I filed an Employee Recourse. I started at the bottom of the ladder 21 and proceeded to take it all the way to Bill Lee.

22 Q. DID YOU FEEL FREE TO EXPRESS ALL OF YOUR CONCERNS?

23 A. Yes.

24 Q. DID YOU EXPRESS ALL OF YOUR CONCERNS?

25 A. Yes.

26 Q. DOES THE DOCUMENT ATTACHED TO YOUR TESTIMONY AS 27 ATTACHMENT A REFLECT YOUR WRITTEN CONCERNS?

4 Q. PLEASE SUMMARIZE AND EXPLAIN WHAT YOU WERE TRYING TO' 5 dOMMUNICATE BY YOUR CONCERNS.

6 A. At the time I submitted my ' concerns, I was trying to show upper 7 management that as a Welding Inspector I cared about my job and 8 the safety of others. After talking with everyone concerned, I was 9 convinced that it was a misund'erstanding on the interpretation of 10 the procedures on my part and theirs. We have since resolved 11 these misunderstandings by proper training in procedures and their 12 interpretation .

13 Q. WERE YOUR CONCERNS INVESTIGATED BY THE TASK FORCES?

14 A. Yes.

15 Q. DID YOU ATTEND ANY MEETINGS WITH TASK FORCE AND/OR QA I 16 MANAGEMENT MEMBERS WHERE THE TASK FORCE FINDINGS, 17 CONCLUSIONS AND RECOMMENDATIONS WERE DISCUSSED?

l 18 A. Yes. I cannot remember details.

19 Q. WERE THERE ANY CHANGES MADE IN THE QA PROGRAM AFTER 20 THE WELDING INSPECTOR CONCERNS AND THE TASK FORCE 21 INVESTIGATION OF THESE CONCERNS?

22 A. Yes.

23 Q. DESCRIBE THE CHANGES OF WHICH YOU ARE AWARE IN THE QA 24 PROGRAM.

25 A. There is now more training in the procedures, and procedures are 26 revised and up to date. There is better understanding by QA 27 Management and great improvement in communications within QA.

1 Q. TO WHNT EXIENT HAVE THESE CHANGES ADDRESSED ISSUES 2 RAISED BY THE WELDING INSPECTOR CONCERNS AND TO WHAT l 3 EXTENT HAVE THESE CHANGES ADDRESSED YOUR PARTICULAR 4 CONCERNS?

5 A. I am satisfied with the quality and safety of the plant. There's a

  • 6 better working relationship among employees. All of my concerns 7 have all been corrected.

8 Q. THE WELDING INSPECTOR CONCERNS HAVE BEEN 9 CHARACTERIZED AS CONCERNS ABOUT THE QUALITY AND 10 SAFETY OF CONSTRUCTION AT CATAWBA. DO YOU AGREE OR 11 DISAGREE WITH THAT CHARACTERIZATION?

12 A. I disagree. The quality of this plant is superior to any other plant 13 that .I know of and the safety precautions are beyond reproach.

14 Q. DID THE EXPRESSION OF YOUR CONCERNS INDICATE YOUR 15 BELIEF THAT THERE WAS A BREAKDOWN IN THE QA PROGRAM 16 OR INDICATE THAT THE QA PROGRAM WAS NO LONGER 17 WORKING?

18 A. While problems did not compromise safety, there were some 19 ccmmunication problems in QA. It seemed as through upper 20 supervision would not always listen to our concerns on issues .

21 Q. DID YOUR CONCERNS REFLECT A BELIEF ON YOUR PART THAT 22 THE CATAWBA PROJECT IS NOT BEING CONSTRUCTED SAFELY?

23 A. No.

24 Q. IN YOUR VIEW, HAS THE QA PROGRAM BEEN EFFECTIVE WHILE 25 YOU HAVE WORKED AS AN INSPECTOR AT CATAWBA?

26 A. Yes.
