Appraisal Appendix A Significant Preparedness Deficiencies Closed (Closed) Open Item (458/8435-04):

The NRC inspector reviewed changes to the Emergency Plan (hereafter referred to as the Plan) in Amendment 16 and the Emergency Plan Implementing Procedures (EIPs).

The Plan and the EIPs appeared to be consistent in designation of the emergency response organization with only minor differences noted.

(Closed' Open Item (458/8435-10):

GSU had established a system assigning

,

qualified personnel to the emergency response organization.

Procedure EPP-2-202, " Emergency Response Organization," under development at the time of the inspection, required that a memorandum approved by the Vice President, RBNG be issued quarterly assigning personnel to the emergency organization.

The first memorandum was approved during the inspection period.

At least two or more individuals had completed required training fcr each position listed.

Those personnel assigned positions in the emergency organization who had.not completed all required training were identified.

The licensee stated that all assigned personnel will complete training by December 1, 1985.

(Closed) Open Item (458/8435-16):

The NRC inspector reviewed training records (computer records) of personnel assigned to the emergency organization.

The records indicated that two or more individuals for each position completed emergency preparedness training as required by EIP-2-102, Attachment 1, " Training, Drills, and Exercises." The licensee stated that the remaining assigned personnel will complete training by December 1, 1985.

Those personnel who had not completed required training had, in general, one or two training modules to complete.

(Closed) Open Item (458/8435-17):

The NRC inspector determined that training program implementation procedure TMP-6-003 had been revised per

" Interim Procedure Change," IPC-6-003-0-1, effective April 2, 1985, requiring that training instructors will, prior to use of any lesson plan or procedure, ensure that the current approved version is used.

A listing of current approved revisions, as well as lesson plans, was being

maintained by the training librarian.

(Closed) Open Item (458/8435-22): The NRC inspector reviewed EIP-2-018,

" Technical Support Center - Activation," and Table 13.3-16 of the Plan and discussed with the supervisor - emergency planning and the training supervisor in alternate substitute for the Emergency Response Information System (ERIS) and the Digital Radiation Monitoring System (DRMS) which were scheduled for completion in February 1986.

EIP-2-018 was revised to provide an alternate substitute for ERIS and DRMS.

If DRMS was inoperable then EIP-2-024, "Offsite Dose Calculation - Manual Method," would be initiated.

If ERIS was inoperable then establish telephone contacts with operations personnel in the main control room to obtain plant parameters and emergency information.

This would be accomplished by using the direct telephone line or over the CR-EOF-TSC hotlin ('

Training of appropriate personnel on EIP-2-024 had been initiated and training on direct line and hot line telephone operation had been provided in Training Module 2, " Communication and Notification."

(Closed) Open Item (458/8435-23):

The NRC inspector determined that ERIS, DRMS, and the Emergency Notification System (ENS) were not operational in the control room.

The augmentation pager system was installed and operational.

Training in the use of the pager system had been completed for 12 of 13 communicator / shift clerks.

Communications training had been completed for 20 of 21 nuclear equipment operators.

GSU's response to this item committed to facility training following main control room completion and turn over as part of the 1985 training cycle to be f

completed prior to. December 1985.

(Closed) Open Item (458/8435-26):

The NRC inspector reviewed the'

emergency response facilities procedures and discussed changes with the supervisor - emergency planning concerning habitability exposure limits to conform with 10'CFR Part 20.

Attachment 1 to EIP-2-012, " Radiation

Exposure Controls," was revised to incorporate limits specified in Part 20.

The 5 rem personnel exposure limit previously contained in the hab~itability monitoring procedures was changed to read, "uninhabitability shall be declared if the radiation exposure rate is greater than

The NRC inspector reviewed EIP-2-016,

" Operation Support Center - Activation," EIP-2-017, " Operations Support Center - Support Function," and Section 13.3.6.1.2 of the Plan.

The

.. inspector determined that EIP-2-016 and EIP-2-017 had been revised to provide the necessary guidance for relocating the operation support center (OSC) in the event of high radiation or airborne activity were such that the OSC became uninhabitable. _ Habitability limits were specified as

400 mR/hr or greater than 4x10 uCi/cc radioiodine.

The technical support center (TSC) was designated as an alternate OSC except for operational personnel who would report to the main control room.

It appeared that the TSC provided adequate space and was designed for habitability under emergency conditions and, therefore, it would be unlikely that the TSC would have to be relocated.

(Closed) Open Item (458/8435-34):

The NRC inspectors verified that the emergency operatiors facility (E0F) layouts were updated and incorporated in figure 13.3.2.2 of the Plan, in Amendment 16 to the FSAR dated February 1985.

The NRC inspector reviewed the sampling and analysis facilities habitability during accident condii. ions and appropriate access procedures for obtaining and analysing liquid effluent samples.

The inspector concurred with the licensee that design of the radwaste liquid waste storage system precluded hazardous liquid waste from being released offsite and that under accident conditions, access to sample locations and sampling could be conducted under routine sampling

The NRC inspector reviewed EIP-2-028 and Section 13.3.8 of Amendment 16 to the Plan and determined that both were substantially the same in requiring release termination to be addressed as a prerequisite to entering recovery.

The NRC inspector toured,the radiological emergency area at the West Feliciana Parish Hospital and discussed facilities, equipment, and training with the Director of Nursing.

It was determined that the facility had been completed, training had been conducted, and supplies and equipment were in a state of readiness.

The NRC inspector noted that EIPs-02-100 and 101 had been revised to control the distribution of the Plan and procedures.

A review of the distribution sign-off sheets showed distribution of the Plan amendment to the State and local governments.

The NRC inspector reviewed communication system records and reports, and held discussions with corporate and RBS staff in regard to the testing of communication systems and correction of identified deficiencies.

Although the. record keeping for periodic communication checks and correction of deficiencies was not finalized, it appeared that identified communications deficiencies had been corrected.

The requirement for periodic tests of'the communication system was contained in EIP-2-102.

The NRC inspector noted that GSU decided not to disseminate the transient public information in the telephone directory based on a cost / effectiveness analysis.

This information will be distributed through the transient brochure and decals.

Section 13.3.5.4.1.2.2 of the Plan has been revised in Amendment 17, dated May 1985, to reflect this change.

A review of training records indicated that 13 of the 16 currently assigned shift supervisors'and shift foremen

had received revised training related to the Plan and emergency classification.

The remaining 3 individuals were in senior reactor operator (SRO) training.

These personnel were expected to complete revised classification training prior to receiving their SRO license.

A walk through emergency exercise was conducted by an NRC inspector with one shift. The walk through indicated this group could effectively detect and classify emergency conditions utilizing the licensee's EIPs.

The NRC inspector noted that the use of checklists, forms, and logs was satisfactory.

(Closed) Open Item (458/8435-106):

The NRC inspector determined that the augmentation radiopaging system and hotline had been installed and were operational.

Notification procedure EIP-2-006 incorporated the use of each system.

Emergency telephone book procedure EIP-2-029 had been approved and provided backup telephone numbers for both the pager and-hotline systems.

As previously noted, most of the control room communicator / shift clerks and nuclear equipment operators had received training.

During the walk through exercise the communicator / shift clerk demonstrated satisfactory knowledge and use of the communications equipment installed in the control room.
