The U.S. Nuclear Regulatory Commission recently received an allegation concerning the adequacy of computer software testing methods at Millstone Unit 3 (RI-89-A-0099). Details of this allegation are enclosed for your review and 6
followup.
We request that the results of your review and disposition of this matter be submitted to Region I within 60 days of the date of this letter. We request that your response contain no personal privacy, proprietary, or safeguards information so it can be released to the public and placed in the NRC Public Document Room. ~1f necessary, such information shall be contained in a separate attachment which will be withheld from public disclosure. The af fidavit required by 10 CFR 2.790(b) must accompany your response if proprietary information is included.
The enclosure to this lett er should be controlled and distribution limited to personnel with a "need to know" until your investigation of the allegation has.
been completed and reviewed by NRC Region I.

ALLEGATION DETAILS The below listed concerns were provided to an NRC inspector:
1.
The Nuclear Engineering and Operations Procedures which provide guidance for the development of Quality Related Computer Programs at the Millstone site are inadequate in that they do not fully implement recognized industry standards.
For example, NEC 2.24 " Quality Software Programs" does not fully adhere to ANS 7.4.3.2 " Application Criteria for Programmable Digital Computer Systems in Safety Systems or Nuclear Power Generating Stations."
2.
The verification / validation process for computer software programs at the Millstone site is deficient since the verification / validation process does not include a functional test with specific predetermined acceptance criteria.
This is contrary to accepted industry practice.
3.
Personnel qualifications for review of software are not specified by procedure. Therefore, the possibility exists for unqualified people to conduct reviews of software packages.
