Oyster Creek Nuclear Generating Station Docket No. 50-219 IE Inspection Report No. 50-219/75-19 In your letter of July 1,1975, an infraction was noted regarding Mr. Plumice's inspection of June 18-20, 1975 at the Oyster Creek Nuclear Generating Station.

This involved the omission of initialing of entries by licensco employees on one or more lines of each of fifty (50) Move Sheets documenting completion and verification of fuel movements performed during the 1975 Refueling Outage.

According to Appendix A of the inspection report, this non-compliance activity was contrary to the requirements of 10CFR50, Appendix "B", Criterion X, Technical Specification, Section 6, Oyster Creek Station Procedures Nos. 212, Refueling and 1002.6.3, SNM Move Sheets.

At all times during refueling operations conducted within the reactor vessel, the refueling crew included a shift foreman (SRO) as a supervisor, a bridge operator to move the fuel, and a checker to verify the fuel moves. Additionally, a licensed operator was stationed in the control room whose refueling responsibilities included being in communication with the refueling crew, verifying the sequence of fuel moves on separate SNM Move Sheets, and monitoring nucicar instrumentation during all periods of fuel The non-compliance cited was a result of not satisfying the require-movement.

In order to avoid a recurrence of this nature, cortective action has been initiated in the following manner:

As part of the Licensed Operator Requalification Program, the applicable refueling procedures and SNM Move Sheet Documentation 4

The purpose of this action is to insure that all of the licensed

operators review the subject procedures and are made aware of i

requirements specified.

The SNM Move Sheet Procedure will be revised to include a requirement that all SNM Move sheets be audited by the Technical Engineering Group to assure that appropriate documentation is i

satisfied.

Item 1 of corrective action is presently underway and Item 2 is i

scheduled to be completed at the conclusion of the next training cycle which is due to commence this fall.

Item 3 will be completed prior to the next scheduled refueling outage.

It is felt that the above corrective actions are sufficient to avoid i

further items of non-compliance in the aret of concern.
