Based on the results of an NRC inspection conducted on November 17-21, l

i 1975, it appears that certain of your activities were not coeducted in full compliance with conditicas of your license as indicated below:

A.

Contrary to 10 CFR 50.59, " Changes, Tests and Experissets,"

i 10 CFR 50, Appendix B, Criterion V, " Instructions, Procedures and Drawings," the Operational Quality Assurance Plan, Section Y j

and reference Procedure No. 105, "Maint===are Repair and 1

l Modification control," and/or Procedure No. 2006 "Modificat3da, Neuroutine Maintenance and Repair," two (2) penetrations providing for cable runs servicing computer banks were drilled adjacent to I

the reactor control room and into the cable spreading room, a i

vital area. This work was completed without required design review and written safety evaluation or formalized work author-ization on or about November 1, 1975.

Contrary to Technical Specification 6.2.A.1 and 6.2.0 and Operating

' pg Procedure No. 304, Revision 0 issued January 22, 1975, Standby Liquid Control valve V-19-24 was observed on November 20, 1975 l

not locked as required and was in addition verified by the cog-nizant shift foreman to be in the open position whereas procedure

304 requires the referenced valve to be closed and locked.

We note that corrective action was taken in that the valve was restored to its proper position and locked, and a revised valve i

lineup checklist was completed November 20, 1975.

l Additionally, procedures do not specifically address the usage and control of combustible materials or special work controls to be effected during welding, nor are provisions specified to provide i

portable or installed fire suppression equipment in work areas where welding or'other ignition processes are conducted.

This is an infraction.
