Description of Item Requiring Resolution:
4 On May 30,-1974, the licensee reported that leakage between an incore monitor housing and the reactor vessel was detected. Leakage was originally indicated on May 28,_1974 during a scheduled hydrostatic test of the-reactor vessel.
On May 29, 1974, a second hydrostatic test was conducted at which time it was determined that the leakage was coming from an incore monitor housing. This indicates a failure of the weld at the inner surface of the vessel-to-housing weld. The licensee has the event under review and has held discussions with the nuc1 car steam supplier and the vessel nanufacturer.
. Repairs will be made by the licensee when their investigation is complete.

i 1.
The Directorate of Regulatory Operations will examine the required repairs to verify that they are made in accordance with code require-ments and qualified procedures.
2.
The Directorate of Licensing will review and evaluate the licensee's report describing their investigation, the repairs and their rationale for resuming operation.
3.
The Directorate of Licensing will determine the need to impose any increases in surveillance requirements or changes to technical speci-fications which may be required.
4.
The Directorate of Regulatory Operations will review the results of any additional surveillance requirements which may be imposed.
5.
The Directorate of Licensing will evaluate this failure with respect to possible generic implications.
