Subject Inspection Report is enclosed for your action. We reconenend

the imposition of a $4,000 civil penalty.

Other enclosures include Carlson's memo to me showing justification

for the penalty, and our recommendations for the wording and format

of the documentation. We modeled the documentation on the September 6,

1973 letter imposing a civil penalty on Theodore R. Schwalm. Inc..

I am of the strong view that a citation for " Failure to Report" has

broad ana beneficial safety implications for Regulatory. A major

fine on this issue should result in more prompt and accurate reports

from all nuclear reactors.
