SUBJECT:
ALLEGED QUALITY ASSURANCE PROGRAMMATIC DEFICIENCIES AT MIDLAND On September 28, 1983, I was approached at the NRC trailers on site by a member of the Midland Plant Quality Assurance Department (MPQAD) to dis-cuss concerns with the identification, documentation, and verification of nonconformances. The individual stated that other members of Quality Assurance / Quality Control had documented nonconformances which they were not allowed by management to address through the use of an NCR.
The alleger could not provide names because either most of the individuals he had talked with wished to remain anonymous, or he had been informed second hand. He felt that QA/QC personnel with outstanding problems should be allowed to input their concerns into the NCR system. He also felt that management should direct QA/QC personnel to input their concerns using the applicable document in accordance with the plant Quality Assurance Manual, CPC-1A.
More recently, the alleger stated that he was told by a member of QA that a Consumers ' NCR associated with ASME requirements was dispositioned without allowing the ANI to implement a hold point as required by the code.
The MPQAD member stated he has been employed on site since May, 1983, and was attempting to inform his management of the lack of control associated with the Nonconformance system. He felt he could receive proper resolution from CPCo QA management by expressing his concerns to the NRC and then inform-ing his management that he had done so.
The alleger provided me with a Quality Action Request (QAR) issued by CPCo which is used to document programmatic nonconformances. Additionally, an in-ternal memo was issued on September 28, 1983, obligating anyone within the Quality Organization who has knowledge of nonconforming conditions to docu-ment such nonconforming conditions, or receive disciplinary actions.
