Steam Generator Feedwater Pump.

The draft has st.atements that are in error.

The pump is driven by a steam turbine, therefore the term " energized" is not appropriato.

Aleo, worn was perIormed, but some work activities. thosc conducted under AWO M2-R9-05948, were not aut,horized by the Operat. ions Department; and. sa r et.y t.ans were not i n s t.a l l ed.

This error, which is the substance of the allegation, apparently occurrea because personnel failed to follow administrat.ive procedures.

The equipment was properly safety targed for overhaul work that was properly authorized.

Additional work, which was re lated, but outsit.e the scope of the overhaul, wan perf ormed under a accond work order without ei ther placing new safety tags for that work order; or, by adding the second work order as a reference to the exist,ing tag authorization.

Steam Generator Feedwater Pump has confirmed that the errors were made it this authorization process. These included inadequate administration of t.he safety tag rlearance to encompass all work activities.

A safe ty boundary for other work on the pump was in place that could have been used while working on the feedwater pump coupling.

Your assertion is therefore true that a safety tag clearance was written for this work but not used and subsequently canceled.

Although the NRC considers that this issue is clooed, we will continue to assecs the effectiveness of the Millstone safety tag program.

I am responding to your December 13, 1990 Millstone Unit 2 questions regarding I&C Department blennial procedure reviews and your concern with maintenance practices associated with main feedwater pump coupling tagout controls. These issues were referred to the licensee for their evaluation. Enclosed for your information is Northeast iltilities' response.

7 Regarding the biennial procedure review, we conclude that the T&C department instructions provide adequate guidance to enable a qualified individual to perform a satisfactory review. In addition, we found that appropriate communications were employed to resolve technical specification - related questions or problems.

- Our evaluation of the tagging for *R" Steam Generator Feed Pump (SGFP) has confirmed that the pump was m fact mistakenly tagged while still energized; however, the corresponding work was not authorized, and no work was therefore performed. Your assertion is therefore true that a clearance was inadvertently written but, nonetheless, was not used and subsequently cancelled.

The NRC considers this issue closed, although we will continue to assess the effectiveness of NStation taEging programs.
