BSEP Unit No. 1 Event Date-November 29, 1982 This event occurred when both the sonic and temperature indications of valve position for SRV-1-B21-F013B and D were inoperable at the same time.

Prior to this event the primary position indication system indicators for these valves, sonic detectors 1-B21-FY-4158 and 4160 were each inoperable, as the indicators' RTGB lights were not functioning.

Routine surveillance during plant operation on November 29, 1982, revealed that the secondary position indication system for these valves tail pipe temperatures indicated by points 2 and 4 on 1-B21-TR-R614 were showing expected indications, but were intermittently printed on the instrument chart paper.

An investigation of the TR-R614 printing problem revealed that wear of the instrument timing motor gear teeth had resulted in sufficient gear to gear mating slippage to prevent consistent printing.

A preliminary investigation of the problems affecting 4158 and 4160 determined the most probable source of the failure 13 with the respective detector to the 4158 and 4160 instruments. As these detectors are located within the drywell, further investigation and a resolution of this problem will be accomplished during the current 1982 Unit No. 1 refueling outage.

The failed timing motor gear in B21-TR-R614 was replaced and the instrument, L&N Speedomax, Model No. P15FAA62, was returned to service.

Present periodic maintenance of this type instrument in this particular application is felt to be adequate in the prevention of future similar events.
