i Conditions Prior to Occurrence _: The unit was in Mode 5, with Power (MWT) = 0, and Load (MWE) = 0.

Description of Occurrence: During the performance of ST 5032.01, ' Monthly Func-tional Test of the Radiation Monitoring System", at 2300 hours0.0266 days <br />0.639 hours <br />0.0038 weeks <br />8.7515e-4 months <br /> on October 21, 1978, it was discovered that Control Room Emergency Ventilation System (EVS) Iso-

lation Damper HV-5301A would not close.

Technical Specification 3.7.6.1 requires two independent Control Room EVS to be operable while the unit is in Modes 1 through 4 Since the unit was in Mode 5 at the time of the occurrence, no action statement was entered. This report is being subnitted as documentation of a component failure.

Designation of Apparent Cause of Occurrence: Inspection of the damper HV-5301A revealed dirt on the sesling surf ace and the seals were in need of lubrication.

These dampers are inspected and cleaned semi-annually per a preventative mainten-ance work order.

Analysis of Occurrence: There was no danger to the health and safety of the public g-or unit personnel. The redundant isolation damper HV-5311A did close as required.

The unit was in Mode 5 at the time of the occurrence.

Corrective Action: Under Maintenance Work Order 78-2476, the damper seats were cicaned and the damper scals were lubricated with silicone spray. The damper was

i stroked and proper operation verified. Surveillance Test ST 5032.01 was performed on October 24, 1978, and the damper operated properly.

It is believed the frequency of the maintenance is adequate, but the damper was not cleaned and lubricated thoroughly. This will be stressed in future work order instructions.

The remaining safety related dampers will be re-inspected, cleaned, and lubricated if required.
