.s Appendix A NOTICE OF VIOLATION Union Electric Company ~

Docket No. 50-483 3ased on the inspection conducted on January 10-13, 23, Feburary 2-3, and 6, 1978, it appears that certain of your activities were in nonccepliance with NRC requirements, as noted below. Items 1 through 4 are infractions, item 5 is a deficiency.

1.

Contrary to 10 CFR 50, Appendix 3, Criteria XV. and the Callaway PSAR, Section 17.1.15, a procedure did not exist to provide an adequate mechanism for closure of nonconfor=ance reports prior to place =ent of concrete. An sxample was provided where a nonconformance report was not closed prior to placement of the first lift for the containment exterior wall. -

2.

Contrary to 10 CFR 50, Appendix 3, Criteria XV, and the Callaway PSAR, Section 17.1.15, a nonconforming condition in which two reinforcing bars had a clear distance between them of less than k

the 1 inch required was not identified during the QC inspection.

Contrary to 10 CFR 50, Appendix 3, Criteria IV-,

and the Callsety PSAR, Section 17.1.4, the technical specification (contract)

No.10456-C-122, Revision 7, for erection of structural steel does not establish cne requirement for veld inspection as indicated in the applicable welding code AWS D-1.1, 1975.

Contrary to 10 CFR 50, Appendix 3, Critaria XVI, and the Callaway PSAR, Section 17.1.16, a deficiency relative to storage of a safety related component was not promptly corrected, although it had been identified on at least two separate occasions by the licensee's inspectors.

5.

Contrary to 10 CFR 50, Appendix 3, Criteria VI, and the Callaway

?SAR, Section 17.1.6, over 100 revisions to safety related drawings were issued for approval by ? aper Calmenson and Company without being checked.
