_ Appendix A NOTICE OF VIOLATION Docket No. 50-483 Union Electric Company Based on the inspection conducted on January 10-13, 23, Feburary 2-that certain of your activities were in 3, and 6. 1976, it appears noncompliance with NRC : 3uirements, as noted below. Items I through 4 are infraccians, item 5 is a deficiency.

1.

Contrary to 10 CFR 50, Appendix B, Criteria XV. and the Callaway PSAR, Section 17.1.15, a procedure did not exist to provide an adequate mechanism for closure of nonconformance reports prior to placement of concrete. An example was provided where a nonconformance report was not closed prior to placement of the first lift for the containment exterior wall. -

Contrary to 10 CFR 50, Appendix B, Criteria XV, and the Callaway 2.

PSAR, Section 17.1.15, a nonconforming condition in which two reinforcing bars had a clear distance between them of less than the 1 inch rer fred was not identified during the QC inspection.

Contrary to 10 CFR 50, Appendix B, Criteria IV, and the Callaway PSAR, Section 17.1.4, the technical specification (contract)

No.10466-C-122, Revision 7, for erection of structural steel does not establish the requirement for veld inspection as indicated in the applicable welding code AWS D-1.1,1975.

Contrary to 10 CFR 50, Appendix B, Criteria XVI, and the Callaway 4

PSAR, Section 17.1.16, a deficiency relc*ive to storage of a safety related component was not promptly corrected, although it had been identified on at least two separate occasions by j

the licensee's inspectors.

Contrary to 10 CFR 50, Appendix B, Criteria VI, and the Callaway PSAR, Section 17.1.6, over 100 revisions to safety related drawings were issued foi approval by Paper Calmenson and Company without being checked.
