APPENDIX A NOTICE OF VIOLATION Nuclear Fuel Services, Incorporated Docket No. 50-201 As a result of the inspection conducted on January 12-15, 1981 and in accordance with the Interim Enforcement Policy, 45 FR 66754 (October 7, 1980), the following violation was identified:
Technical Specification 7.1.4 states, in part, that detailed written procedures shall be provided for operation of the plant.
In ad.iition, procedures for radiation protection shall be provided.
Section 8.0 of the facility Health and Safety Procedures Manual developed pursuant to Technical Specification 7.1.4 in paragrah 5.2.a states, in part, "Obtain the required information to fill out the Conditional Release Form.
The information required is discussed below...." This information included, but was not limited to principal isotope, activity, transport group, material form, and, type of label applied.
Contrary to the above, on December 29 and 30, 1980, Conditional Release Forms prepared for the shipment of the NFS-4B and NFS-4A casks, respectively, were not completed to record the information stated above as required.
This is a Severity Level VI Violation (Supplement VI.F).
Pursuant to the provisions of 10 CFR 2.201, Nuclear Fuel Services, Incorporated is hereby required to submit to this office within twenty-five days of the date of this Notice, a written statement or explanation in reply, including:
(1) the corrective steps which have been taken and the results achieved; (2) corrective steps which will be taken to avoid further violations; and (3) the date when full compliance will be achieved.
Under the authority of Section 182 of the Atomic Energy Act of 1954, as amended, this response shall be submitted under oath or affirmation.
