f [TTi'l I fuse from panel Y-1 blew. The procedure did not contain a warning that terminal 12 was the 118 VAC supply to the SFAS cabinet. Mod T-5361 was written to the procedure.

TOLEDO EDISON COMPANY UDAVIS-BESSE NUCLEAR POWER STATION UNIT ONE SUPPLEMENTAL INFORMATION FOR LER NP-33-81-05 DATE OF EVENT: January 19, 1981 FACILITY: Davis-Besse Unit 1 IDENTIFICATION OF OCCURRENCE: Loss of power to Safety Features Actuation System (SFAS) Channel 1.

3 Conditions Prior to Occurrence: The unit was in Mode 5 with Power (MWT) = 0 and l Load (Gross MWE) = 0.

Description of Occurrence: On January 19, 1981 at 1550 hours0.0179 days <br />0.431 hours <br />0.00256 weeks <br />5.89775e-4 months <br />, during the performance of the SEAS Grounding Test, TP 520.02, power was lost to SFAS Channel 1 cabinet. This j made SFAS Channel l containment radiation inoperable. This placed the unit in.the

Action Statement 9 of Technical Specification 3.3.2.1, which required the inoperable '

unit to be placed in the tripped condition within one hour. The containment radiation bistable trips upon loss of pcwer.

j Designation of Apparent Cause of Occurrence: Instrument and Control personnel were ,

installing a j umper between terminal- 12 of TB121 and instrument ground. - When the j jumper was installed, an arc occurred, and the supply fuse from panel Y-1 blew. The

cause of the event was a proced 33 deficiency. The test procedure, TP 520.02-and Con-3 solidated Controls Corporation test outline did not give any warning that terminal 12 was the 118 VAC supply to the SFAS cabinet.

Analysis of Occurrence: There was no danger to the-bealth-and safety of the public or

! to station personnel. The remaining three channels of SEAS containment radiation were *

operable during this time. _

i j Corrective Action: The fuse was replaced under Maintenance Work Order 81-1356 and 1 power was restored at 1710 hours0.0198 days <br />0.475 hours <br />0.00283 weeks <br />6.50655e-4 months <br /> the same day. Temporary modification T-5361 was 1 written to TP 520.02 to provide adequate warning.

Failure Data: A similar occurrence in which a procedure deficiency caused a loss of an SEAS channel was reported in Licensee Event-Report NP-33-80-59.
