Based on results of the NRC inspection condur.ted on June 20-23,1978, it appears that certain of your activities were not conducted in full com-pliance with Appendix B to 10 CFR 50 as indicated below:

Failure to Follow Pipino Installation Procedures F

Criterion V of Appendix B requires that established instructions, pro-cedures, or drawings be followed for all activities affecting quality.

[i.:

Brown und Root Instruction 35-1195-PCI-001, " Pipe Department Cleanliness Control Guidelines," requires that each pipe spool, after being welded to previously completed piping runs, be examined by a pipe fitter fore-man for cleanliness, capped, taped with red tape and tagged.

Contrary tc the above:

The inspector observed on June 21 and June 22, 1978, that the unwelded cnds of four pipe spools in the lower two headers of the containment spray system were capped but not taped or tagged as required.
