'o LF.R A'1TACHMENT '- RO ill-31-26 Facility: BSEP Unit.N'o.1 Event Date: 2-14-81 This event and the one reported in LER 2-81-09 occurred because a spring-loaded pin on the i.echunical slip en cennection between the ficw paddle and transmitter of the ficw indicator / recorder became loose due to vibration in the effluent discharge line causing an inaccurate ficw signal to be sent to the ficw recorder. In each case, the slip-en connection was reassembled with a new pin, the flow indicator / recorder and transmitter were checked for calibration and systeo was returned to normal operation. As both of these events occurred due to the same problem, it is believed that another transmitter of different design with its accc=panying mechanical connection hardware will eliminate the source of the problems encountered.

Plant preventative maintenance procedures are being revised to calibrate this indicator / recorder and transmitter on a quarterly basis. It is believed that this will help reduce the nu=ber of system f ailures until a permanent fix is in effect.
