, -l i DATE OF EVENT: December 8, 1977 FACILITY: Davis-Besse Unit 1 i-IDENTITICATION OF OCCURRENCE: Control Room Normal Ventilation placed in recircula-tion mode due to chlorine detector failure Conditions Prior to Occurrence: The unit was in Mode 1, with Power (MWT) = 1118, and Load (MWE) = 380.

I Description of Occurrence: on December 8, 1977 at 1045 hours0.0121 days <br />0.29 hours <br />0.00173 weeks <br />3.976225e-4 months <br />, Surveillance Test 4

ST 5037.02, " Chlorine Detection System Functional Test", was performed. During

! the course of the test, AE-4863A, the chlorine detector at the tank car was tripped and could not be reset. This isolated the Control Room Normal Ventilation

- System. In order to utilize the larger capacity of the Normal Ventilation System, AE-4863A was jumpered out and the Control Room Normal Ventilation System was thought to have been placed in the recirculation mode at 1645 hours0.019 days <br />0.457 hours <br />0.00272 weeks <br />6.259225e-4 months <br />. However, Operations personnel failed to close HV-5036, the Control Room Outside Air Intake Damper. ,Upon further investigation, it was found that AE-5358A, the Control Room Ventilation Air Intake Chlorine Detector, was inoperable.

Cd AE-4863A, AE-535SA and their associated isolation dampers' form one chlorine detec-4 tion / Control Room Isolation System string. The redundant string consisting of detectors AE-4863B, AE-5358B and their associated isolation dampers were operable.

4 The occurrence placed the unit in Action Statement A of Technical Specification .

At 2300 hours0.0266 days <br />0.639 hours <br />0.0038 weeks <br />8.7515e-4 months <br /> on December 8,1977, it was found that HV-5306 was open. This defeated the recirculation mode of operation, placing the unit in violation of Action State-ment A of Technical Specification 3.3.3.7 from 1645 hours0.019 days <br />0.457 hours <br />0.00272 weeks <br />6.259225e-4 months <br /> to 2300 hours0.0266 days <br />0.639 hours <br />0.0038 weeks <br />8.7515e-4 months <br /> on December 8, 1977. . It was not realized until the Station Review Board Meeting on January 4, 1978, that damper HV-5306 had not been initially closed on December 8 at 1645 hours0.019 days <br />0.457 hours <br />0.00272 weeks <br />6.259225e-4 months <br />.

HV-5306 was closed at 2300 hours0.0266 days <br />0.639 hours <br />0.0038 weeks <br />8.7515e-4 months <br />, placing the unit in compliance with Action State-ment A of Technical Specification 3.3.3.7.

Designation of Apoarent Cause of Occurrence: The apparent cause of damper IIV-5306 being open was operator error. .The damper should have been manually closed from the Control Room co place the Control' Room Normal Ventilation System in the recircula-tion mode at 1645 hours0.019 days <br />0.457 hours <br />0.00272 weeks <br />6.259225e-4 months <br /> on December 8,1977. Operations personnel had failed to do this. At 2300 hours0.0266 days <br />0.639 hours <br />0.0038 weeks <br />8.7515e-4 months <br /> on December 8, 1977, HV-3306 was found in the open position i and was immediately closed.

The apparent cause of-the failures of both AE-4863A and AE-5358A was construction /

installation error. ' AE-4863A was found to have a knock-out for an electrical connection in its electrolyte reservoir. The resulting corrosion clogged the felt

. wick and destroyed the chemical properties of the electrolyte. The printed circuit board in detector AE-535SA had some dirt on it which was believed to have prevented 1

proper operation. .

The redundant chlorine detection / Control Room isolation string consisting of AE-4863B, AE-5358B and associated isolation dampers was opera-ble throughout the occurrence. The Control Room Normal Ventilation System was in the recirculation mode after HV-5306 was closed at 2300 hours0.0266 days <br />0.639 hours <br />0.0038 weeks <br />8.7515e-4 months <br /> on December 8, 1977.

Had a chlorine leak occurred at any time, the Control Room would have been isolated by the redundant detection / isolation string.

Correcti.ve Actien: At 2300 hours0.0266 days <br />0.639 hours <br />0.0038 weeks <br />8.7515e-4 months <br /> on December 8,1977, upon finding damper HV-5306 in the open position, Operations personnel closed it i==ediately. .This placed the Control Room Normal Ventilation System in the recirculation mode and removed the unit from violation of Action Statement A of Technical Specification 3.3.3.7. A seminar will be held with Operations personnel to emphasize the necessity of l closing HV-5306 when placing the Control Room Normal Ventilation in the recircula-tion mode.

On detector AE-4863A, the chlorine detector at the tank car, the knock-out was re-moved from the reservoir, and the felt wick, electrode and electrolyte replaced.

It was declared operable at 1230 hours0.0142 days <br />0.342 hours <br />0.00203 weeks <br />4.68015e-4 months <br /> on December 10, 1977.

detector AE-5358A of the Control Room Ventilation Air Intake, the felt wick was eplaced and the orifice and electrode cleaned and the circuit board cleaned and inspected. At 1500 hours0.0174 days <br />0.417 hours <br />0.00248 weeks <br />5.7075e-4 months <br /> on December 11, 1977, Surveillance Tests ST 5037.01,

" Chlorine Detection System Channel Calibration" and ST 5037.02, " Chlorine Detec-tion System Functional Test", were completed satisfactorily.

This proved the operability of all parts of the chlorine detection / Control Room' Iso-lation System and removed the unit from Action 3tatement A of Technical Specifica-tion 3.3.3.7.

At 1030 hours0.0119 days <br />0.286 hours <br />0.0017 weeks <br />3.91915e-4 months <br /> on December 12, 1977, the Control Room Normal Ventilation System was placed in the normal mode. A work request has been issued to investigate the re-dundant detectors for stailar conditions. -
