DATE OF EVENT: January 16, 1978 FACILITY: Davis-Besse Unit 1 IDENTIFICATI0N'0F OCCURRENCE: Decay Heat Pump 1-1-failed to start.

Conditions Prior to Occurrence: The unit was in Mode 2, with Power (MWT) = 0, and Load DME) = 0.

' Description of Occurrence: At 1000 hours0.0116 days <br />0.278 hours <br />0.00165 weeks <br />3.805e-4 months <br /> on January 16, 1978 when performing Sur-veillance. Test ST 3051.01, " Emergency Core Cooling System (ECCS) Subsystems Monthly -

Test", Decay Heat Pump 1-1 failed to start. This placed the unit in the Action Statement of Technical Specification 3.5.2 which requires two operable Decay Heat /

Low Pressure Injection Pumps in Modes 1, 2 and 3.

Designation of Apparent Cause of Occurrence: The fuses in the breaker for the Decay i h Heat Pump 1-l motor which supply control power to the close (start) circuit were

/ not making proper contact. The exact reason for the fuses not making proper contact ts not known at present.

Analysis of Occurrence: Therc was no danger to the health and safety of the public or to unit personnel. Decay Heat Pump 1-2 was operable. Reactor power was less ** ~

than 1% of full power.

Corrective Action: At 1008 hours0.0117 days <br />0.28 hours <br />0.00167 weeks <br />3.83544e-4 months <br /> on January 16, 1978, the control power fuses were pulled.and then re-inserted into the fuse block. The Surveillance Test was success-fully completed which returned Decay Heat Pu=p 1-1 to service. This removed the unit from the Action Statement of Technical Specification 3.5.2. Maintenance Work 1

Order 78-193 has been issued to inspect fuses and fuse blocks to assuru proper contact.

Failure Data: No previous _similar events have occurred.
