- s TOLEDO EDISON COMPANY DAVIS-BESSE NUCLEAR POWER STATION UNIT ONE SUPPLEMENTAL INFORMATION FOR LER NP-33-80-37 DATE OF EVENT: April 6, 1980 FACILITY: Davis-Besse Unit 1 IDENTIFICATION OF OCCURRENCE: Independent Off-Site AC Sources Lined up and Availabic, ST 5080.01, was Missed Conditions Prior to occurrence: The unit was in Mode 1, with Power (MRT) = 2024, and Lord (Gross FME) = 647.

Description of_0ccurrence: On April 6, 1980, the Shift Supervisor was reviewing the computer printed surveillance test schedule and could not find an entry for a weekly test, ST 5080.01, Independent Off-Site AC Sources Lined Up and Available, which is normally run on Sundays. When reviewing the Unit Log, he found that this test had not been run between March 23, 1980 at 0430 hours0.00498 days <br />0.119 hours <br />7.109788e-4 weeks <br />1.63615e-4 months <br /> and April 3, 1980 at 0930 hours0.0108 days <br />0.258 hours <br />0.00154 weeks <br />3.53865e-4 months <br />.

Using the Surveillance Requirement 4.0.2 frequency criteria, the late date passed on April 1, 1980 at 0030 hours3.472222e-4 days <br />0.00833 hours <br />4.960317e-5 weeks <br />1.1415e-5 months <br />. This meant the station had missed the Surveillance Rcquirement 4.8.1.1.1.a which requires the o't-site AC sources to be determined operable at least once per seven days by verifying correct breaker alignments and indicated power availabilities. The test was run again on April 6, 1980 and again l varified the sources operable.

Designation of Apparent Cause of Occurrence: The cause of the occurrence was an error mtde when updating the computer test history. The test run March 23, 1980, was pro-cessed through the Technical Section where it was recorded in a log book. Houever, it was missed when the completed test information was updated into the computer. When thm next schedule was printed up, the test due March 23 continued to show up as due.

The computer program will not call for a March 30 test until the test done March 23 h o been entered as complete. When the Shift Supervisor saw the March 23 test on the cchedule, he knew from checking the ir.ic log that it had already been done, and he -

crossed out this entry.  ;

This error went undetected during.a time that the Surveillance Test Engineer's duties ,

wzre being turned over to a new individual. Also, the Surveillance Test Engineer's  ;

ctudent assistant had just started the previous week.  ;

I Anelysis of Occurrence: There was no danger to the health and safety of the public or to station personnel. The test was performed only two days late, and when done, it has c nsistently shown the off-site AC sources to be operable.

i Corrective Action: The test has been performed on' April 3 and 6, 1980, since the missed.

The duties of the Surveil-Icnce Test Engineer have been reviewed with the new Surveillance Test Engineer and the tew student assistant to ensure the computer schedules are more closely checked and errors c:rrected before they become significant.
