TOLEDO EDISON COMPANY DAVIS-BESSE NUCLEAR POWER STATION UNIT ONE SUPPLEMENTAL INFORMATION FOR LER NP-33-79-145 DATE OF EVENT: December 8, 1979 FACILITY: Davis-Besse Unit 1 IDENTIFICATION OF OCCURRENCE: Decay Heat Cooler 1-1 Component Cooling Water Out-let Valve CC1467 would not stroke from the Control Room.

Conditions Prior to Occurrence: The unit was in Mode 5, with Power (MWT) = 0, and Load (Gross MWE) = 0.

Description of Occurrence: On December 8,1979 at 2300 hours0.0266 days <br />0.639 hours <br />0.0038 weeks <br />8.7515e-4 months <br /> during the performance of Surveillance Test ST 5074.01, Component Cooling Water (CCW) System Monthly Test, Decay Heat Cooler 1-1 CCW Outlet Valve CCl467 would not stroke from the Control Room switch. Technical Specification 3.5.3.c requires one emergency core cooling subsys-tem operable comprised of one operable decay heat cooler in Mode 4. Since the sta-tion was in Mode 5 at the tbne of the occurrence, this incident is being reported as documentation of a component f ailure. The valve was manually opened.

Designation of Apparent Cause of Occurrence: The cause of the occurrence was that the mechanical 1inkage in the valve had vibrated loose. This has been determined to be a design problem with the linkage connecting pin of the valve operator.

Analysis of Occurrence: There was no danger to the health and safety of the public or to station personnel. Decay Heat Cooler 1-2 was operable at the time of the occurrence.

Corrective Action: On December 9,1979 per Facility Change Request 79-347, a design change was made to the linkage under the direction of the manufacturer. The linkage connecting pin was locked in place to prevent a loosening of the linkage from vibra-tion. The valve was declared operable on December 10, 1979 after the successful completion of ST 5074.01.

Failure Data: There have been four previously reported occurrences with failures of this type actuator. They were reported in Licenset Event Reports NP-33-78-120, NP-33-78-147, NP-33-79-74 and NP-33-7 9-ll4.
