TOLEDO EDISON COMPANY DAVIS-BESSE NUCLEAR POWER STATION UNIT ONE SUPPLEMENTAL INFORMATION FOR LER NP-33-80-06 DATE OF EVENT: January 12, 1980 FACILITY: Davis-Besac Unit 1 IDENTIFICATION OF OCCURRENCE: Ventilator fire damper blocked open with fiberglass insulation Conditions Prior to Occurrence: The unit was in Mode 1, with Power (MWT) = 2764, and Load (Gross MWE) = 923.

Description of Occurrence: On January 12, 1980 at 2200 hours0.0255 days <br />0.611 hours <br />0.00364 weeks <br />8.371e-4 months <br />, an equipment opera-tor on his rounds found fiberglass insulation stuffed into an open wall ventilator between the Water Treatment Building and the Service Water Valve Room on the 565' elevation. This ventilator was recently equippped with a three hour rated fire damper per Facility Change Request 78-083 Supplement No. 33. The fiberglass insu-lation would have prevented the damper from closing, therefore, the damper was in-operable. This entered the unit into the action statement of Technical Specifica-tion 3.7.10 which requires a fire watch be established with!r one hour. The opera-tor immediately removed the insulation returning the damper to an operable condi-tion and removing the unit from the action statement.

Designation of Apparent Cause of Occurrence: The cause of this event was personnel error in that an unknown persen placed the fiberglass in the damper to block a cold air draft blowing through the area which inadvertently blocked the fire damper as well.

Analysis of Occurrence:

There was no danger to the health and safety of the public or to unit personnel. The Service Water Valve Room sprinkler system was operable and would tave protected any safety related equipment that may have been exposed to a fire passing through the inoperable ventilator damper.

Corrective Action: The insulation was removed from the damper at 2200 hours0.0255 days <br />0.611 hours <br />0.00364 weeks <br />8.371e-4 months <br /> on January 12, 1980 removing the unit from the action statement of Technical Speci-fication 3. 7.10. A memo to al.1 station personnel, security, and project management was prepared to warn about blocking fire dampers.

Failure Data: There have been no previously reportable occurrences of this type.
