Analysis of Occurrence:

On Nuvember 7, 1979, the required operability tests were performed prior to removing #2 diesel generator from service for annual inspection. Tecnnical Specifications 4.5.F.1, surveillance re-quirement, states: "When it is determined that one diesel gen-erator is inoperable, all low pressure core cooling and containment cooling subsystems shall be demonstrated to be operable immediately and daily thereafter. In addition, the operable diesel generator shall be demonstrated to be operable immediately and daily there-after."

The required operability test on #1 diesel generator was conducted again on November 8, 1979.

On November 9, 1979, the 0800 to 1600 operating crew performed the required surveillance procedures for the low pressure core cooling and containment cooling subsystems during the first half of the shift. Following performance of these operability tests, it was believed that #2 diesel generator was ready to return to service.

The operability test for #2 diesel generator was started during the second half of the shift and, if satisfactorily completed, would have deleted the necessity to perform the operability test of #1 diesel generator. However, a problem with diesel generator #2 occurred and that unit was not made operable on that shift. The 0800 to 1600 operating crew failed to then perform the required surveillance for #1 diesel generator. The relieving shift, 1600 to 2400, noting that the required surveillance for the low pressure core cooling and containment cooling subsystems had been performed also failed to recognize that #1 diesel generator operability had not been performed.

When the 0800 to 1600 shift returned to work on the morning of November 10, 1979, they noted #2 diesel generator had not been returned to service and that the required operability for #1 diesel generator had not been performed the previous day.

This occurrence presented no adverse consequences fram the stand-point of public health and safety.

Corrective Action:

On the morning of November 10, 1979 at approximately 0800 hours0.00926 days <br />0.222 hours <br />0.00132 weeks <br />3.044e-4 months <br />, the Operations Supervisor was informed of the failure to perform the operability test on #1 diesel generator on November 9, 1979.

The required operability test for #1 diesel generator was immed-iately performed satisfactorily.

A Shift Supervisor Turnover Checklist is being developed and will be in use by January 1, 1980 as a part of TMI Lessons Learned work.

Use of this checklist should reduce the chances of errors of this nature.

This occurrence has been discussed directly with the operations personnel involved and all shift supervisors. A copy of the non-conformance report has been routed to all licensed personnel.

Additionally, a copy of this LER will be routed to all licensed personnel.
