OPS~G Public Service Electric and Gas Company P.O. Box E Hancocks Bridge, New Jersey 08038 Salem Generating Station Mr. William T. Russell Regional Administrator USNRC Region I 631 Park Avenue King of Prussia, Pa. 19406
Dear Sir:
June 22, 1989 TECHNICAL SPECIFICATION ACTION 3.7.10.1.b.2.b NOTIFICATION SALEM NO. 1 DOCKET NO. 50-272 On June 22, 1989 at 0250 hours0.00289 days
0.0694 hours
4.133598e-4 weeks
9.5125e-5 months
, the automatic actuation feature for both*Fire Pumps was found deactivated.
The sensing line between the lFPlO and 2FP10 fire pump cross over valves was found isolated thereby precluding the automatic initiation function.
The No. 2 Fire Pump had recently been-returned to service on June 19, 1989 at 2150 hours0.0249 days
0.597 hours
0.00355 weeks
8.18075e-4 months
.
It had been removed from service in support of replacement of the jockey Pump and associated isolation valves.
When the pump was returned to service, an operability test was successfully performed.
This test requires the subject sensing line to be connected.
Therefore, between the time of test completion and 0230 hours0.00266 days
0.0639 hours
3.80291e-4 weeks
8.7515e-5 months
on June 22, the sensing line was isolated.
The Hope Creek Generating Station water system has been in service since April 1989.
This system acts as the backup fire suppression system for Salem Station.
Upon discovery of this event, the lFPlO valve was opened in order to line up the pressure switch used for pump automatic actuation.
This notification is being made in compliance with Technical Specification Action Statement 3.7.10.1.b Section 2.b.
As required by Section 2.c, of the Action Statement, a written "Special Report" will be forthcoming within fourteen (14) days of this event.
