Auxiliary Feedwater Pump Manual Trip During Safeguards Equipment Control System Operation CONDITIONS PRIOR TO OCCURRENCE:

Cold Shutdown DESCRIPTION OF OCCURRENCE:

On December 4, 1980, during routine surveillance testing, it was found that the Nos. 11 and 12 Auxiliary Feedwater Pumps could be stopped from the control console if a steam generator low low level is received during Safeguards Equipment Control System operation.

This is a malfunction of a safety-related system and is a violation of Technical Specification 6.9.l.8e.

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

Design Error ANALYSIS OF OCCURRENCE:

Manual operation of the Nos. 11 and 12 Auxiliary Feedwater Pumps should not be possible during Safeguards Equipment Control System operation.

CORRECTIVE ACTION

DCR l-EC-1084 was issued and implemented on December 6, 1980, to preclude manual shutdown of the Auxiliary Feedwater Pumps during Safeguards Equipment Control System operation for Unit 1.

DCR 2-EC-1085 implemented on February 24, 1981 will accomplish the same for Unit 2.
