1. 13 Steam Generator Channel II Steam Flow Failed High CONDITIONS PRIOR TO OCCURRENCE:

Mode 2 Startup _ 8 Rx Power 5 x 10 A

DESCRIPTION OF OCCURRENCE:

While performing routine startup operations, #13 Steam Generator Channel II steam flow failed high.

Action Statement 3.3.1.1-7 was entered at 0540 hours0.00625 days <br />0.15 hours <br />8.928571e-4 weeks <br />2.0547e-4 months <br /> which allows continued startup and/or power operation to ~ontinue until performance of the next required channel functional test provided the inoperable channel is placed in the tripped condition within one hour.

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

Equipment Malfunction ANALYSIS OF OCCURRENCE:

Upon investigation, it was determined that the Multiplier/Divider Unit (Hagan Model 112) in the channel had failed.

CORRECTIVE ACTION

The defective Hagan Module was replaced with a spare.

The replacement unit was calibrated and a satisfactory functional test performed.

The Action Statement was terminated at 1147 hours0.0133 days <br />0.319 hours <br />0.0019 weeks <br />4.364335e-4 months <br /> on December 23, 1980.
