Both Fire Suppression Water Systems Inoperable.

CONDITIONS PRIOR TO OCCURRENCE:

Unit Nos. 1 and 2 -

Mode 5 -

Cold Shutdown No. 1 Fire Pump was inoperable since 0530 hours0.00613 days <br />0.147 hours <br />8.763227e-4 weeks <br />2.01665e-4 months <br />, October 12, 1980.

No. 2 Fire Pump was operable on the fire suppression header and backup fire suppression system was supplied from Hope Creek site by opening 1FP30.

DESCRIPTION OF OCCURRENCE:

During flushing of the fire suppression header, the header pressure dropped below the starting pressure for the No. 2 Fire Suppression Pump and the pump started but was automatically shutdown after a 90 second run.

This was observed twice and Action Statement 3.7.10.lb was entered at 1415 hours0.0164 days <br />0.393 hours <br />0.00234 weeks <br />5.384075e-4 months <br /> on October 23, 1980.

The alternator was found to have failed and was replaced.

The fire pump was auto started and run for fifteen minutes.

The No. 2 Fire Pump was declared operable and the Action Statement was terminated at 0230 hours0.00266 days <br />0.0639 hours <br />3.80291e-4 weeks <br />8.7515e-5 months <br /> on December 4, 1980.

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

Equipment Failure ANALYSIS OF OCCURRENCE:

The alternator output is used to supply a successful auto start signal.

The failure of the alternator resulted in an auto shutdown after a 90 second time delay.

CORRECTIVE ACTION

The alternator was removed and replaced.
