.Loss of Containment Integrity CONDITIONS PRIOR TO OCCURRENCE:

Refueling Operations Mode 6 DESCRIPTION OF OCCURRENCE:

While the 13MS169 and 13MS171 valves were disassembled for maintenance, containment integrity was lost due to the grinding out of a weld at the No. 13 Steam Generator feedwater nozzle.

The condition was dis-

  • covered when it was noticed that the ground out weld was sucking air.

Action Statement 3.9.4c was entered at 1645 hours0.019 days <br />0.457 hours <br />0.00272 weeks <br />6.259225e-4 months <br /> on October 28, 1980.

The 13MS171 valve was reassembled and closed and the 13MS168 valve was positioned to it.

This restored containment integrity and the Action Statement was terminated.at 1746 hours0.0202 days <br />0.485 hours <br />0.00289 weeks <br />6.64353e-4 months <br />".

Personnel Error ANALYSIS OF OCCURRENCE:.

The loss of containment integrity allows the possible uncontrolled release of the containment atmosphere to the environment.

CORRECTIVE ACTION

This LER will be circulated to the appropriate people to illustrate the need for considering the consequences of concurrent jobs even if they are seemingly unrelated.
