Loss of Audio Count Rate Signal CONDITIONS PRIOR TO OCCURRENCE:

• DESCRIPTION OF OCCURRENCE:

Action Statement 3.9.2 was entered at 1930 hours0.0223 days <br />0.536 hours <br />0.00319 weeks <br />7.34365e-4 months <br /> on October 28, 1980, because the audio count rate was lost.

The audio count rate drawer was replaced with the Unit 2 audio count rate drawer.

The action Statement was terminated at 2040 hours0.0236 days <br />0.567 hours <br />0.00337 weeks <br />7.7622e-4 months <br />.

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

A broken card holder in the audio count rate drawer caused the loss of audio signal.

The card holder broke when the drawer was removed.

The cables that hang down in the cabinet caught the edge of the circuit board when the drawer was removed.

ANALYSIS OF OCCURRENCE:

The loss of the audio count rate signal prevents the audio neutron level count rate (i.e. change in core reactivity) from being heard by personnel in the control room or containment.

CORRECTIVE ACTION

The broken card holder was replaced and the audio count rate drawer was returned to operation.

Signs are being installed warning personnel to insure that the cables clear the circuit boards when they are removing the drawers.
