Fuel Handling Area Ventilation System Inoperable CONDITIONS PRIOR TO OCCURRENCE:

Mode 5 Cold Shutdown DESCRIPTION OF OCCURRENCE:

At 1157 hours0.0134 days <br />0.321 hours <br />0.00191 weeks <br />4.402385e-4 months <br /> on September 23, both Fuel Handling Building exhaust ventilation fans were observed smoking.

Upon investigation, it was discovered that the #11 exhaust fan belts were missing and the #12 belts were smoking.

At 1200 hours0.0139 days <br />0.333 hours <br />0.00198 weeks <br />4.566e-4 months <br />, both exhaust fans were declared inoperable and the Action Statement for Technical Specification 3.9.12 was entered.

The belts on #12 were replaced with an unmatched pair because only one belt was available in stock.

The #12 fan was returned to service and the Action Statement was terminated at 2045 hours0.0237 days <br />0.568 hours <br />0.00338 weeks <br />7.781225e-4 months <br /> on September 23, 1980.

At 0305 hours0.00353 days <br />0.0847 hours <br />5.042989e-4 weeks <br />1.160525e-4 months <br /> on September 27, the #12 FHB exhaust fan belts were observed slipping.

The fan was taken out of service and the Action Statement was again entered.

The belts on the #11 exhaust fan were replaced with a matched pair obtained from a Unit 2 Fuel Handling Building exhaust fan and the Action Statement was terminated at 1542 hours0.0178 days <br />0.428 hours <br />0.00255 weeks <br />5.86731e-4 months <br /> on September 27, 1980.

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

Equipment failure compounded by the lack of proper spare parts.

  • ANALYSIS OF OCCURRENCE:

Initially, the V-belts failed for an unknown reason.

Apparently, the belts on #11 fan had failed prior to September 23 but were not discovered until the failure of #12 fan.

The belts on #12 were replaced by an unmatched pair of V-belts because a matched pair were unavailable.

Because of the mismatch, this pair failed four days later.

LER 80-51/03L 2 -

CORRECTIVE ACTION

The V-belts were replaced with a matched pair.

An Outstanding Item (81-80-01) was issued to the Office Administrator to resolve the* problem of availability of matched pairs of belts.
