Missed Surveillance CONDITIONS PRIOR TO OCCURRENCE:

Mode 3 Hot Standby DESCRIPTION OF OCCURRENCE:

During a 'record review of previous surveillances in preparation for a reactor startup, it was determined that the required inservice inspection surveillance of valves per Technical Specification 4.0.S had not been completed as required at the previous reactor shutdown.

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

Procedure Inadequacy ANALYSIS OF OCCURRENCE:

The surveillance in question must be performed at least each 92 days

  • when in Mode 3, Hot Standby, but cannot be performed while in Mode l~

power operation.

If a reactor shutdown occurs and the surveillance has not been completed within the previous 92 days, it must be per-formed prior to startup.* At present, however, there is no way that performance of this surveillance is flagged during the period between a reactor shutdown and subsequent startup as the scheduling is done on a calendar basis.

CORRECTIVE ACTION

The required surveillance was completed immediately upon identifying the need.

Operating Instruction I.... 3.3, "Hot Standby to Minimum Load", has been corrected by On-the-Spot Change P-11 and is presently being revised to contain an entry in the appropriate mode change checklist to insure that this surveillance is performed in a timely manner.
