At 1810 hours0.0209 days <br />0.503 hours <br />0.00299 weeks <br />6.88705e-4 months <br /> on September 8, 1980, during a functional test, a failed detector was dfscovered in the Air Particulate Detector (APD).

Equipment Failure ANALYSIS OF OCCURRENCE:

The detector was found to be failed due to a cracked sodium iodide crystal.

The cause of the crystal failure is not known.

The Action Statement was terminated at 2145 hours0.0248 days <br />0.596 hours <br />0.00355 weeks <br />8.161725e-4 months <br /> on September 8, 1980.

CORRECTIVE ACTION

The detector was replaced and tested satisfactorily.
