Containment Air Lock Inoperable CONDITIONS PRIOR TO OCCURRENCE:

Mode 1 Reactor Power 85%

DESCRIPTION OF OCCURRENCE:

During a routine containment entry, it was determined a roller was broken on the 100' elevation outer door.

Action Statement 3.6.1.3 was entered at 1550 hours0.0179 days <br />0.431 hours <br />0.00256 weeks <br />5.89775e-4 months <br /> which requires with an air lock inoperable, restore the air lock to operable status within 24 hours2.777778e-4 days <br />0.00667 hours <br />3.968254e-5 weeks <br />9.132e-6 months <br /> or be in at least hot standby within the next six hours and in cold shutdown within.the following 30 hours3.472222e-4 days <br />0.00833 hours <br />4.960317e-5 weeks <br />1.1415e-5 months <br />.

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

Equipment Failure ANALYSIS OF OCCURRENCE:

over a period of time, the roller has been hit by the door latch resulting in failure of the roller.

CORRECTIVE ACTION

The roller was replaced in kind from stock and tested satisfactorily.
