Failed Seismic I Systems Pipe Hangers CONDITIONS PRIOR TO OCCURRENCE:

Operational Mode 5 DESCRIPTION OF OCCURRENCE:

During inspection of pipe hangers on Seismic I systems in accordance with NRC Bulletin 79-14, three hangers on the Safety Injection and Chemical and Volume Control Systems were found to have a broken weld, broken bolt and a broken pipe guide.

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

The broken pipe guide and the broken hanger weld were determined to be caused by unusual pulsations from No. 13 Charging Pump due to an inlet poppet valve failure causing additional loadings on these supports.

The broken bolt was due to an unidentified construction problem..

• ANALYSIS OF OCCURRENCE:

The piping systems in which deficiencies were found and the repairs made were evaluated and included in our response to IE Bulletin 79-14.

CORRECTIVE ACTION

An inspection of the Charging Pump identified the poppet valve failure.

• work Orders were issued to repair the deficient supports and the poppet valve.

A vibration dampener, Design Change 1EC590, was added to the system to prevent recurrence.

The repairs have been inspected in service and found to be satisfactory.
