No. 14 Steam Generator Steam Pressure Channel IV Inoperable CONDITIONS PRIOR TO OCCURRENCE:

Operational Mode 1, Reactor Power 99%

DESCRIPTION OF OCCURRENCE:

At 0845 hours0.00978 days <br />0.235 hours <br />0.0014 weeks <br />3.215225e-4 months <br />, a steam line differential pressure alarm on the status panel showed Lqop 1 steam pressure lower than Loop 4.

A check of steam line pressure channels showed normal readings for all channels except Loop 4 Channel IV which read greater than 100 psi above normal pressure.

The channel was declared inoperable and Action Statement No. 14 for Technical Specification 3.3.2.1 was implemented.

At 1020 hours0.0118 days <br />0.283 hours <br />0.00169 weeks <br />3.8811e-4 months <br />, the channel was returned to service and the Action Statement was terminated.

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

The cause of this occurrence was a frozen sensing line to No. 14 Steam Generator Steam Pressure Transmitter.

An open outside ventilation duct located above the transmitter cabinet directed cold air onto the sensing line.

Space heaters for this area were not energized.

ANALYSIS OF OCCURRENCE:

Technical Specification 3.3.2.1 Action 14 requires that with the number of operable channels one less than the total number of channels, operation may proceed until the performance of the next required channel functional test, provided the inoperable channel is placed in the tripped condition within one hour.

The channel was placed in the tripped condition updn implememtation of the Action Statement and was returned to service within 1 hour1.157407e-5 days <br />2.777778e-4 hours <br />1.653439e-6 weeks <br />3.805e-7 months <br /> and 35 minutes.

CORRECTIVE ACTION

The air stream from the ventilation duct was redirected away from the transmitter cabinet and the space heaters were energized.

Design Change 1SC107 has been completed which installed a closable damper in the ventilation duct. *No further cor~ective action is required.
